Cornerstone Staffing
CSC No. 44132-R3
In accordance with the Professional Services Agreement entered into between Smith
Temporaries, dba: CornerStone Staffing ("Contractor") and the City of Port Worth
("City") effective on or about December 1, 2012, City Secretary Contract ("CSC") No.
44132, (collectively the "Professional Services Agreement"), this Appendix A-4A is
incorporated into the Professional Services Agreement and hereby reads as follows:
1. The term of this Work Order shall be extended to end when all services have
been completed under this work order as contemplated herein, but not later
than the end date set forth below.
2. This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement.
3. The services to be performed under this Work Order shall be as follows:
MANAGEMENT ANALYST II — ERP
1. Determine appropriate accounting treatment and record financial transactions
in the General Ledger
2. Monitor and reconcile general ledger accounts
3. Perform complex financial analysis for various programs within the city
4. Prepare financial reports and audit schedules
5. Provide assistance during the City's independent financial and single audits
6. Perform other related accounting/finance duties and responsibilities as
required.
4. Personnel assigned to provide services under this Work Order:

Start Date: December 1, 2015
End Date: November 30, 2016
Payment Terms:
Payment Rate: $56.00 per hour, inclusive of expenses
5. This Appendix A-4A shall terminate immediately in the event the contract
personnel does not begin to provide contract services to the City under this
Appendix as of the Start Date stated above. If the contract personnel begins
to provide services, but Contractor provides notice to the City that the contract
personnel will no longer provide services under the Appendix, and such

notice is effective prior to the End Date stated above, then this Appendix shall
terminate immediately as of the effective date of the notice from Contractor
