Professional Services Agreement between the City of Fort Worth ("City") and The Process
Advisors
City Secretary Contract ("CSC)No. 45836 (the "Agreement")
Renewal Term: August 15, 2015 through August 16, 2016.
The above-referenced Agreement, which expired on August 14, 2015, has two renewal options. Pursuant
to the Agreement, renewals are at the City's option. This letter is to inform you that the City requests
renewal of CSC No. 45836 for an additional one year period, which will begin immediately and will end
on August 16, 2016. All other terms and conditions of CSC No. 45836 remain unchanged. Please return
your signed agreement letter, along with a copy of your current insurance certificate, to the address
set forth below.
