Discovery Benefits,Inc.
and the
City of Fort Worth
1.0 The City of Fort Worth hereby amends the above-referenced contract.
1.1 Add Section 8.26 INTERLOCAL PURCHASING as follows:
8.26 INTERLOCAL PURCHASING AGREEMENTS (applicable to competitively procured
goods/services contracts):
A. The City of Fort Worth has entered into Interlocal Purchasing Agreements with
other governmental entities, pursuant to the Interlocal Cooperation Act,Chapter 791 of the Texas Government Code.
The Contractor amends its response to RFP 15-0222 Health Benefits Management to state "Yes" to cooperative
purchasing.
The Contractor offers the same prices and terms and conditions to other eligible governmental agencies that have an
interlocal agreement with the City of Fort Worth.
B. The City of Fort Worth does not accept any responsibility or liability for the
purchases by other governmental agencies through an interlocal cooperative agreement.
2.0 The total contract amount is increased by$0.00 for the term. The total Contract authorization is recapped
below:
