Memorandum of Understanding—Implementation Phase
K16 Ready Society,Inc.,DBA, Texas Initiative Programs
AND
City of Fort Worth
This In-Kind Memorandum of Understanding is hereby entered into for the purpose of
participating in the implementation process for the K16 Ready Society,Inc.,DBA,Texas
Initiative Programs (Lead Agency/Fiscal Agent)Byrne Criminal Justice Innovation project.
The success of the K16 Ready Society,Inc., DBA, Texas Initiative Programs implementation
process is greatly dependent on strong collaboration and highly engaged partnerships with local
stakeholders, community members and project partners. As such, K16 Ready Society,Inc.,
DBA, Texas Initiative Programs is convening a Strategic Planning Committee to help guide the
project,inform the process and help fmalize the K16 Ready Society,Inc.,DBA,Texas
Initiative Programs implementation plan for the Byrne Criminal Justice Innovation project that
will target zip code 76112.As a the primary law enforcement agency of the community and key
partner to the project we would like to invite you to participate in this K16 Ready Society, Inc.,
DBA,Texas Initiative Programs Strategic Planning Committee.
RESPONSIBILITIES AND EXPECTATIONS
For this project K16 Ready Society,Inc.,DBA, Texas Initiative Programs serves as the
"lead" organization. As the lead organization,K16 Ready Society, Inc.,DBA, Texas
Initiative Programs is responsible for the following:
• Convening the K16 Ready Society,Inc., DBA, Texas Initiative Programs Strategic
Planning Committee.
• Coordinating implementation activities for the K16 Ready Society,Inc.,DBA, Texas
Initiative Programs project, including but not limited to organizing focus groups,
interviews of key stakeholders, etc.
• Facilitating communications and information sharing with the K16 Ready Society,Inc.,
DBA, Texas Initiative Programs Leadership Council.
• Drafting and completing the K16 Ready Society, Inc.,DBA, Texas Initiative Programs
project implementation plan.
• Measuring key outcomes and indicators for K16 Ready Society, Inc., DBA, Texas
Initiative Programs implementation.
• Overseeing the implementation of the K16 Ready Society,Inc.,DBA, Texas Initiative
Programs project over the three year grant period. �o AA 12 Ap

Under this Agreement, City of Fort Worth agrees to:
• To provide crime and arrest data needed to complete the crime analysis for this program
in addition to specific staff that will oversee this effort.
• Participate in quarterly meetings over the grant period if possible and only if staffing
allows (October 1, 2016—October 1, 2019)
TIME PERIOD
This Memorandum of Understanding shall follow the grant time period,beginning upon signing
this document and remaining in place through October 1,2019.
TERMINATION
This Agreement may be terminated in whole or in part by either party without cause. Written
notice of termination shall be given in writing to both the K16 Ready Society, Inc.,DSA, Texas
Initiative Programs and City of Fort Worth and shall be sent via certified or registered mail
with return receipt requested. Failure to honor any of the obligations stated above may also result
in the termination of this Agreement
