In accordance with the Professional Services Agreement signed between Kforce, Inc.
("Contractor") and the City of Fort Worth ("Client" or "City") effective as of November 6,
2013 and approved by M&C C-26551 on November 5, 2013 (the "Agreement"), this
Appendix B-6 is incorporated into the Professional Services Agreement and hereby
reads as follows:
1. The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein, but not later
than the end date set forth below.
2. This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement, specifically the services to be provided
under Section 1 of said agreement.
3. The services to be performed under this Work Order shall be for IT Personal
Computer and peripherals support and installation as follows:
IT PC Support Technician
a) Install, upgrade and perform maintenance on PC hardware and peripheral
components on the City's enterprise network
b) Load and verify correct operation of all application software packages and
operating systems used throughout the City's data network, such as operating
system, word processing, spreadsheet programs, proprietary departmental
software and enterprise-wide software.
c) Provide training and technical assistance to users
d) Identify and resolve hardware, software, and operator problems.
e) Make minor repairs to Desktops, Laptops and/or Printers or replace any
Customer Serviceable Units (designated by the Original Equipment Manufacturer
customer replicable/repairable units or assemblies), or refer to services personnel
for repair or replacement.
f) Document work in IT Service Management System.
g) Move assets from delivery site to end user site and ensure connectivity for
refreshed hardware and software. Remove End of Life PC hardware from end
5V6 user and return to City location for appropriate disposition.

h) Work as a resource on the citywide hardware and software refresh project or
other projects such as a Department relocating to an alternate temporary or
permanent location.
i) Perform inventory management and warehouse operations with regard to City
owned IT equipment.
j) Assist City staff with various support needs of Information Technology Solutions
as assigned, such as receiving new hardware delivered to a designated City
location, placing specific hardware items aside for future installation for an end
user and preparing excess property hardware and peripherals for formal
disposition.
k) This position is required to participate in a 24x7x365 on-call rotation schedule for
departments such as Police, JEOC, Water, Code and other departments that
operate on a 24x7 schedule.
4. Personnel assigned to provide services under this Work Order:

End Date: October 31, 2016
Payment Terms:
Payment Rate: $39.95 per hour
5. This Appendix B-6 shall terminate immediately in the event the contract
personnel does not begin to provide contract services to the City under this
Appendix as of the Start Date stated above. If the contract personnel begins
to provide services, but Contractor provides notice to the City that the contract
personnel will no longer provide services under the Appendix, and such
notice is effective prior to the End Date stated above, then this Appendix shall
terminate immediately as of the effective date of the notice from Contractor.
