Renewal Term No. 2: September 1, 2016 to August 31, 2017
The above referenced Contract will expire on August 31,2016. Pursuant to the Contract, renewals are at the mutual
agreement of the parties. This letter is to inform you that the City requests renewal of CSC No45861 for an additional
one year period, which will begin immediately upon the expiration of the current term and will end on August 31,
2017. Pricing for the agreed upon term shall be$9,438.00. All other terms and conditions of CSC No.45861 remain
unchanged.
Please return three (3)signed agreement letters, along with a copy of your current insurance certificate,to
the address set forth below.
