TO CITY SECRETARY CONTRACT No. 44636 WHEREAS, the City of Fort Worth (CITY) and Dunaway & Associates, L.P. , (ENGINEER) made and entered into City Secretary Contract No. 44636, (the CONTRACT) which was authorized by M&C C-26319 on the 9th day of July, 2013 in the amount of $ 387, 787. 00; and WHEREAS, the CONTRACT was subsequently revised by Amendment Number 1 in the amount of $43, 785 . 00 which was administratively authorized on July 9, 2014; and WHEREAS, the CONTRACT involves engineering services for the following project : Bellaire Park Court Drainage Improvements; and WHEREAS, it has become necessary to execute Amendment No. 2 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated September 1, 2015, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $22, 705. 00. (See Attached Funding Breakdown Sheet, Page -3-) 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $454, 277 . 00 . 3 . All other provisions of the Contract, which are not expressly amended herein, shall remain in full force and effect . City of Fort Worth EFT. City of Fort Worth Transportation& Public Works Department Fort Worth, Texas 76102 Reference: Amendment No. 2 Bellaire Park Court Drainage Improvements City Project No. 01081 City Secretary Contract No.44636 Ms. Willhelm: This letter constitutes our request for supplemental authorization for tasks outside the scope of services of our current contract. The additional work includes the following tasks: General 1. Design Management and Coordination —This includes team management, QA/QC, and issuing plan changes for pavement replacement and culvert modifications. Includes one additional meeting with the City. 2. Full Pavement Replacement for Bellaire Drive South —This includes developing pavement section and paving details for Bellaire Drive South, Major Arterial section. Pavement sections will be developed for rigid and flexible pavement. Proposed design will match existing slopes and tops of curb of Bellaire Drive South, therefore Plan- Profile, Jointing Layout, or Roadway Cross-Section sheets will not be generated. It is anticipated that four(4) additional plan sheets will be created. 3. Full Pavement Replacement for Bellaire Park Court— Design will include preparation of the Typical Flexible Pavement Section and Details for the residential street. Proposed design will match existing slopes and tops of curb of Bellaire Park Court from the cul- de-sac northward to Bellaire Drive South, therefore Plan-Profile or Roadway Cross- Section sheets will not be generated. It is anticipated that one (1) additional plan sheet will be created. On 8-20-2015, added Rigid Pavement Section and Details, revised Pavement Layout, and added Trench Drain details. It is anticipated that one (1) additional plan sheet will be created. 4. Overton Park Retaining Wall Modifications—This consists of the following items: Amendment No. 2 Bellaire Park Court Drainage Improvements June 1, 2015 Page 2 a. A pedestrian-rated rail or special fence details to be provided by PACSD to provide a barrier between the general public and the vertical drop at the outfall structure in the park. This rail or fence will be mounted on the top or behind the retaining wall constructed at the drainage outfall. b. Insert in the plans, suitable notes and references for approved surface finish, color, and form pattern of all exposed concrete surfaces as selected by PACSD. c. Insert in the plans, suitable notes and references to require approval of the size, color, and physical characteristics of the stone used to protect slopes at the outfall structure. These modifications will require coordination with PACSD to obtain details, approved products, and related information. This work will modify one or more existing plan sheets and up to two (2) additional plan sheets to be created. 5. Temporary Sanitary Sewer Conflict Box at Overton Park Drive E. —This includes design modifications to the existing cast-in-place culvert section to allow the existing 24" Sanitary Sewer (M-189) to remain in service during construction. The intent of the design is to allow M-189 to remain in service while a replacement 30" sanitary sewer is constructed, after which time the 24" pipe in conflict will be removed and the walls of the concrete cast-in-place repaired. These modifications will require four(4) plan sheets to be modified. 6. Roadway Control Sheet and Elevations—This includes preparing a layout of Bellaire Drive South and Bellaire Park Court to include existing back of curb information. Please note that this does not a redesign of the pavement along the storm drain alignment. Intervals. An ASCII point file will be provided including Northing, Easting, Elevation and Description for each point. It is anticipated that two additional plan sheet will be created.
