Cover Page 2 DOCUMENT NO. 7560005286 -01 ATTACHMENT NO. 03 PERFORMING AGENCY: FORT WORTH DEPARTMENT OF PUBLIC HEALTH RECEIVING AGENCY PROGRAM: TEXAS DIABETES PROGRAM TERM: September 01, 2000 THRU: August 31, 2001 SECTION I. SCOPE OF WORK: PERFORMING AGENCY shall design, implement and evaluate a Diabetes Awareness and Education in the Community (DAEC) Project. The DAEC project will target Hispanic Americans, African Americans, the elderly and other groups at risk for developing Type 2 Diabetes. PERFORMING AGENCY shall comply with all applicable federal and state laws, rules, regulations, standards, and guidelines in effect on the beginning date of this contract Attachment unless amended. PERFORMING AGENCY shall develop the following products: • an assessment of PERFORMING AGENCY'S capacity to design and implement a DAEC project; • a community-wide environmental scan; and • a plan of action for local enhancement of the DAEC project during the term of this contract Attachment. PERFORMING AGENCY is responsible for designing , implementing, and evaluating the DAEC project. PERFORMING AGENCY shall meet the following objectives through the DAEC project and shall provide evidence that the DAEC project is meeting the following objectives: • implement staffing patterns with qualified staff designated to the DAEC project; • reach consensus between PERFORMING AGENCY and the community on community- wide priorities that describe population-based objectives; achieve compatibility with the Healthy Communities 2000 Model Standards; • increase PERFORMING AGENCY'S credibility within the community, and establish a "specific efforts and accomplishments" (SEA) performance system; • identify and recruit program champions within the organizational structure and the community; • change significant systems in PERFORMING AGENCY'S organizational structure and its efforts in the community to improve organizational capacity and performance; • increase staff development within the organizational structure and in the community; • improve PERFORMING AGENCY'S infrastructure and performance; • increase access to health care and education services from baseline; and • achieve established workload measures by reaching the negotiated number of persons to be served through this contract Attachment through awareness activities. ATTACHMENT - Page 1 PERFORMING AGEISTCY'S DAEC project shall provide evidence that PERFORMING AGENCY is meeting end-of-project outcome objectives: • increase visibility of the DAEC to at least 15% of the target population; and • increase the community's understanding of diabetes by at least 10% from baseline. All activities shall be performed in accordance with RECEIVING AGENCY'S Program guidelines as set out in the Request for Proposal (RFP) issued March 6, 2000, and with PERFORMING AGENCY'S objectives, activities work plan, detailed budget and revisions, if any, as adopted by RECEIVING AGENCY Program and transmitted in writing to PERFORMING AGENCY. These documents are incorporated by reference and made a part of this contract Attachment. PERFORMING AGENCY shall comply with the following: • Chapter 103 of the Health and Safety Code; • Healthy People 2000, National Health Promotion and Disease Prevention Objectives. U.S. Department of Health and Human Services, September, 1990; • RECEIVING AGENCY'S Quality Care: Client Services Standards for Public Health and Community Clinics. Texas Department of Health, revised June 1997; and • Diabetes Today. A Course on How to Build Skills to Plan and Implement Community Based Programs for Persons with Diabetes. U.S. Department of Health and Human Services, December 1993. Within thirty (30) days of receipt of an amended standard(s) or guideline(s), PERFORMING AGENCY shall inform RECEIVING AGENCY Program, in writing, if it will not continue performance under this Attachment in compliance with the amended standard(s) or guideline(s). RECEIVING AGENCY may terminate the Attachment immediately or within a reasonable period of time as determined by RECEIVING AGENCY. PERFORMANCE MEASURES The following performance measure(s) will be used to assess, in part, PERFORMING AGENCY'S effectiveness in providing the services described in this contract Attachment, without waiving the enforceability of any of the other terms of the contract. Number of printed articles - 36 Number of radio spots - 6 Number of television spots - 2 Number of media exposures - 240,000 Number of community changes - 42 The numbers above indicate the minimum for each performance measure. This value is subject to change upon written agreement between RECEIVING AGENCY and PERFORMING AGENCY. PERFORMING AGENCY shall submit monthly programmatic reports within fifteen (15) days after the end of each month that are prepared in the format approved by RECEIVING AGENCY Program. PERFORMING AGENCY shall also submit quarterly programmatic reports within fifteen (15) days after the end of each quarter that are prepared in the format approved by RECEIVING AGENCY Program. ATTACHMENT - Page 2 SECTION II. SPECIAL, PROVISIONS: PERFORMING AGENCY shall include the following credit on all printed, audio or visual materials developed as part of this program: "Funding provided by the Diabetes Program, Texas Department of Health." PERFORMING AGENCY shall immediately notify RECEIVING AGENCY Program Director, Diabetes Program, of any changes in project activities or staff. ATTACHMENT - Page 3 / 0 SECTION III. Authorize the City Manager to apply for, and if offered, accept and execute a grant contract with the Texas Department of Health (TDH) and the Texas Diabetes Program/Council (TDP/C) for $100,000 to be used to increase diabetes awareness and education at the community level; and 2. Adopt the attached appropriation ordinance increasing appropriation's and estimated receipts by $100,000 in the Grants Fund; and 3. Apply indirect cost at a rate of 27.31%, which represents the rate that was budgeted during the last fiscal year of this grant proposal, and is the agreed upon rate currently being applied to the grant. An increase to the indirect cost would significantly impair the possible refunding of this grant. DISCUSSION: Diabetes is an epidemic and a public health crisis in Texas. It is estimated that the population in Texas with diabetes is 6.5%. Diabetes impacts minorities disproportionately, with 11.4% of African-Americans and 9.3% of Hispanics suffering from the disease (Texas Diabetes Fact Sheet, 1997). In 1998, the City's Public Health Department Community Needs Assessment showed that the prevalence of diabetes in Fort Worth was 8.8%. In order to address the prevalence of diabetes, the TDH and the TDP/C offer the Diabetes Awareness and Education in the Community (DAEC) grant. The primary objective of the DAEC grant is to increase diabetes awareness through education and prevention-oriented programs. The Public Health Department was one of 10 programs selected by the state for the first two years of the DAEC grant from FY1998-2000, and is applying for the third year continuation of this program. On June 9, 1998 (M&C C-16822), the FY1998-1999 grant was approved, and on July 27, 1999 (M&C C-17554,) the FY1999-2000 grant was approved. `. -The Public Health Department has used several different strategies in the past two years to raise awareness in the community. These included community and corporate health fairs, presentations to concerned citizens and neighborhood organizations and media campaigns which included radio, television-and print. a e. , ti City of Fort Worth, Texas "111jor and council Communication DATE REFERENCE NUMBER LOG NAME PAGE 5/16/00 **C-18004 50THIRD 2 of 2 SUBJECT APPROPRIATION ORDINANCE AND GRANT CONTRACT WITH THE TEXAS DEPARTMENT OF HEALTH AND THE TEXAS DIABETES PROGRAM/COUNCIL FOR A DIABETES AWARENESS AND EDUCATION IN THE COMMUNITY GRANT FOR THE THIRD YEAR FROM SEPTEMBER 1, 2000 TO AUGUST 31, 2001 The Public Health Department plans to continue these strategies in the following year with the addition of the following: educating staff at day-care centers, elementary schools and nursing homes on healthier meal options; educating doctors on the importance of teaching their diabetic patients about life long self-management and developing community groups that will assist in disseminating diabetes awareness. FISCAL INFORMATION/CERTIFICATION: The Finance Director certifies that upon acceptance of the grant contract and adoption of the attached appropriation ordinance, funds will be available in the current operating budget, as appropriated, of the Grants Fund.
