Consultants, L.L.C. (Engineer) made and entered into City Secretary Contract No. 263561 (the Contract) which was authorized by the City Council by M&C C-18297 on the 17th day of October 2000; and WHEREAS, the Contract involves engineering services for the following project: , Engineering contract for Water and Sanitary Sewer Replacement Contract 2000 WSM-E (Water and Sewer Maintenance) . WHEREAS, it has become necessary to execute Amendment No. 1 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated December 22, 2000, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $13,977.42. 2. Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $153,023.42 . -2- F-��ti� l VO"1H, 0" 3. GPC S:\.I0B\000640\TEXT\CHAUBEY.LTR Excellence in Engineering Exhibit"A" SCOPE OF SERVICES Gutierrez, Smouse, Wilmut & Assoc., Inc. (GSW) proposes to furnish engineering services to ANA Consultants (ANA) to perform Internal Television Inspection services as specified below for an evaluation of the Construction performed by Southwestern Underground on portions of the City of Fort Worth sanitary sewer system. The following is a clarification of the tasks that the ENGINEER will perform under ATTACHMENT A. Upon receipt of Notice to Proceed, the ENGINEER will perform the following tasks: I. STUDY AREAS The sanitary sewer mains for evaluation are located throughout the City of Fort Worth. The estimated footage for Internal Television Inspection is 7,059 linear feet (L.F.) of sanitary sewer mains. II. SCOPE OF WORK Task 1 - Internal Television Inspection and Cleaning Without Dye Flooding This task consists of preparing selected sewer lines for internal television inspection and performing inspection on an estimated 7,059 L.F.. Of these lines, 3,530 L.F..shall be cleaned to provide for an unobstructed internal inspection. Cleaning will be accomplished by making two (2) passes with a hydraulic cleaning unit(jet truck). Sludge, mud, sand, gravel, and other sediment and debris shall be removed from the manhole and disposed from both upstream and downstream manholes. If an obstruction cannot be removed, the line segment will be turned over to the City for assistance. GSW will receive no extra compensation for attempting cleaning in both directions. Internal television inspection (closed-circuit, colored picture) of the selected sewer lines is to determine the extent of construction defects and the necessary rehabilitation. The lines selected for television inspection are shown in the attached schedule for the New Composite Contract. Video tapes will be made of all lines televised(VHS format). The location of the line, size, direction of flow, etc. will be noted at the beginning of each tape by written text and audio. Written documentation will also be provided which identifies the segment by upstream and downstream manholes, states length and size, and notes by station (distance)all defects, service line taps, root intrusion, grease, infiltration, etc. All lines to be televised will be plugged if necessary to eliminate incoming flows from upstream collection lines. Compensation for these tasks will be on a "per linear foot" basis. Task 2 -Administration on Sub-Consultant This task is part of ANA's contract. III. COMPLETION TIME The Assignment can be initiated immediately upon acceptance of this Agreement by both GSW and ANA.
