WHEREAS, the City of Fort Worth (City) and APM and Associates,
Inc. , (Engineer) made and entered into City Secretary Contract No.
28672, (the Contract) which was authorized by the City Council by
M&C C-19560 on the 29th day of April 2003; and
WHEREAS, the Contract involves engineering services for the
following project:
Water and Sanitary Sewer Replacement Contract 2002; STM-N
WHEREAS, it has become necessary to execute Amendment No. 1 to
said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:

Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated November
29th, 2004, a copy of which is attached hereto and incorporated
herein. The cost to City for the additional design services to be
performed by Engineer total $17,295.00.
2.
Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $140,810.00.

All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Dear Mr. Chaubey:
APM & Associates, Inc. is submitting this fee proposal for Amendment No. 1 to the contract for the above
referenced project based on additional services requested during the design phase. The following is a brief
description and cost for the additional services provided on this project:
1. Additional Design of approximately 1,400 LF of water and sanitary sewer replacement in N.W. 27"
Street.
Subtotal N.W. 27th St. Design: $ 10,470.00
2. Additional Surveying and Basemapping of approximately 2,100 LF for the replacement
along N.W. 27th St. and N.W. Loraine St.
Subtotal Surveying: $ 6,825.00
Total Contract Amendment No. 1 $ 17,295.00
For additional services provided pursuant to this Amendment No. 1, we request compensation in an
amount not to exceed Seventeen Thousand, Two Hundred Ninety-Five Dollars ($17,295.00). Such
payment shall be made in accordance with the payment terms specified in the Contract.

Rakesh,
The original NW 27th Street segment being designed by APM went from Sherman Street to McCandless Street.
Another consultant was designing NW 27th Street from Robinson Street south. The section between McCandless
Street and Robinson Street not being designed by anyone was added to the APM segment.
If you want an exhibit, I can fax you a figure for it.
