AMENDMENT AND RENEWAL NO. 1 TO
CITY SECRETARY CONTRACT NO.47466
AGREEMENT FOR UNEMPLOYMENT COST MANAGEMENT SERVICES
This Amendment and Renewal is hereby entered into by and between the City of Fort
Worth (hereafter "City"), a home rule municipal corporation situated in portions of Tarrant,
Denton and Wise Counties, Texas, with its principal place of business at 1000 Throckmorton
Street, Fort Worth, Texas, and Corporate Cost Control, Inc. (hereafter "Consultant"), a
corporation acting by and through Ajah Anderson, its duly authorized representative, each
individually referred to as a"parry"and collectively referred to as the"parties."
WHEREAS, the parties have previously entered into City of Fort Worth City Secretary
Contract No. 47466 (the "Contract") for the purpose of unemployment cost management
services, which was last executed on January 29, 2016 with an effective date of November 1,
2015, and
WHEREAS,the original term of the Contract expires on October 31, 2015; and
WHEREAS, the City and Consultant wish to amend the Contract to incorporate three (3)
successive one-year renewals; and
WHEREAS, the City and Consultant now desire to renew the Contract through October
31, 2017 with an effective date of November 1, 2016.
NOW, THEREFORE, City and Consultant, acting herein by and through their duly
authorized representatives, enter into the following agreement to amend the contract:

The first line of the section entitled "Agreement Duration and Payment Schedule" shall read as
follows:
Agreement Duration and Payment Schedule.
The "Initial Term" of this agreement shall be effective for one year from November 1, 2015,
unless terminated by either party in accordance with this agreement. After expiration of the
Initial Term, the City may renewal this contract for three (3) additional successive one-year
terms, each a"Renewal Term" with thirty (30) days' written notice to the Consultant. Extension
of the term shall not require nor allow an increase of Consultant's Fee under the Contract unless
approved in writing by both Parties.

The Parties agree that the Contract is hereby extended for an additional one-year Renewal Term
with an effective date of November 1, 2016. After execution of the first renewal, the Contract
has two (2) additional successive one-year options to renew.
3.
All other provisions of the Contract which are not expressly amended herein shall remain
in full force and effect.
