PROFESSIONAL SERVICES AGREEMENT
This AMENDMENT TO CITY SECRETARY CONTRACT NO. 47457,
PROFESSIONAL SERVICES AGREEMENT is made and entered into by and between the
CITY OF FORT WORTH, a municipal corporation of Tarrant, Johnson, Denton, and Wise
Countries, Texas, acting herein by and through Fernando Costa, its duly authorized Assistant
City Manager, hereinafter referred to as the "City," and TEXAS COALITION FOR ANIMAL
PROTECTION("Contractor"), a Texas non-profit corporation, each individually referred to as
"party" and collectively as "parties".
RECITALS
WHEREAS, the City of Fort Worth ("City") and TEXAS COALITION FOR
ANIMAL PROTECTION ("Contractor"), made and entered into City Secretary Contract No.
47257 (the "Agreement") on January 28, 2016; and
WHEREAS, the Agreement describes a scope of services for Veterinary Services as
described in Attachment A-Scope of Services and originally included a six month term; and
WHEREAS, staff recommends amending the Agreement to extend the term with
Contractor from the original six month term for an additional eighteen months with a total
compensation of$50,000.00 for the amended term; and
NOW THEREFORE, City and Contractor acting herein by and through their duly
authorized representatives enter into the following that amends the Agreement:
1.
The City and the Contractor agree to amend City Secretary Contract No. 47457 to extend
the term in Section 2 for eighteen months from the expiration of the Agreement to terminate on
July 28, 2017 and to amend Section 3 to provide for a total compensation of$50,000.00 for the
extended term as provided by this Amendment. The Agreement, as amended, may be terminated
in accordance with Section 4 of the Agreement.
2.
All other provisions of the Agreement which are not expressly amended herein shall
remain unchanged, including the scope of services and fees.
