FIRST ADDENDUM TO THE FIRST AMENDMENT 46710-A1 TO CITY SECRETARY
CONTRACT NUMBER 46710 MASTER PROFESSIONAL SERVICES AGREEMENT
WITH STRATEGIC GOVERNMENT RESOURCES, INC
This addendum to the first amendment CSC No. 46710-A 1 to the City Secretary Master
Professional Service Agreement, City Secretary Contract Number 46710, is made and entered
into by and between THE CITY OF FORT WORTH, a home-rule municipal corporation of the
State of Texas ("City"), acting by and through its duly authorized Assistant City Manager, and
STRATEGIC GOVERNMENT RESOURCES, INC. ("Consultant" or "SGR"). A Texas
corporation acting by and through it duly authorized Managing Director.
WHEREAS,the City and Consultant,entered into that certain agreement identified as City Secretary
Contract No. 46710 for a term beginning June 2, 2015 and ending June 1, 2016; and
WHEREAS,such Contract provided for four(4)consecutive one(1)year renewal options,provided
all other conditions remained the same as detailed in the Contract; and
WHEREAS,the parties have exercised its first renewal option under City Secretary Contract 46710
and;
WHEREAS,the parties amended the Agreement by City Secretary Contract Number 46710-A 1 and
now wish to execute an addendum this Amendment; and
NOW,THEREFORE,for and in consideration of the mutual covenants contained herein,and other
good and valuable consideration,the receipt and sufficiency of which are hereby acknowledged and
confessed, the parties hereto agree as follows:
I. First Addendum.
The parties hereby wish to exercise the first addendum to City Secretary Contract
Amendment 46710-A1, effective November 18, 2016. The parties agree that the other
renewal options remain in full force.
II. EXHIBIT A-1, Scope of Work.
The parties hereby agree to delete the Name: AMI Strategies and Solutions, LLC at
paragraph 6 on page 5. Further, the parties agree that Consultant Personnel assigned to
provide services under this agreement shall read"Name: Gerald Lee Chandler and Richard
Dale Davis".

III. Other Terms and Conditions.
All other terms and conditions of the Agreement remain in full force and effect,except that if
there is a conflict between the Agreement and this Addendum, this Addendum shall control.
IV. Counterparts.
This Extension and Amendment may be executed in counterparts(but all counterparts,taken
together, shall constitute only one agreement)and facsimile or electronic copies of signatures shall
be treated as originals.
