Contract No.CSC No.43586-ADI (the"Contract")
Renewal Term 2: September 14,2016 to September 13,2017
The above referenced Contract expired on September 14,2016.Pursuant to the Contract,contract renewals are at the sole option of the
City.This letter is to inform you that the City is exercising its right to renew CSC No.43586-ADI for an additional one year period,
which will begin immediately upon the expiration of the current term and will end on September 13, 2017. All other terms and
conditions of 43586-ADI remain unchanged. Please return this signed acknowledgement letter, along with a copy of your
current insurance certificate and three original signed copies to the address set forth below, acknowledging receipt of the
Notice of Contract Renewal.
Please log onto BuySpeed Online at http://fortworthtexas.gov/purchasing to insure that your company information is correct and up-
to-date. Once fully executed,a copy of the contract renewal will be sent to Boopsie Inc in order to generate an invoice.
