("Consultant"). WHEREAS, City and Consultant entered into an Agreement identified as Fort Worth City Secretary Contract("CSC")No. 46820 with an Effective Date of April 6, 2015; WHEREAS, City and Consultant extended the Agreement through the execution of CSC No. 46820-R1, for a Renewal Term commencing on April 6, 2016, and expiring on April 6, 2017; WHEREAS, the Agreement did not include additional renewal terms; however, the City and Consultant desire to amend the Agreement to add two additional Renewal Terms; and WHEREAS, it is also the collective desire of both City and Consultant to amend the Agreement to add the additional services of adding Spanish language alerts as outlined in the attached Exhibit A; NOW THEREFORE, City and Consultant, acting herein by and through their duly authorized representatives, enter into the following agreement effective February 15, 2017: 1. Fort Worth CSC No. 46820, as renewed and amended through CSC No. 46820-R1, is hereby amended by modifying Section 2 of the Agreement, as modified through CSC No. 46820-R1, to add two additional Renewal Terms to the Agreement. Such Renewal Terms shall be from April 6, 2017, through April 5, 2018, and April 6, 2018,through April 5,2019, and may be exercised at the City's option. 2. Fort Worth CSC No. 46820, as renewed and amended through CSC No. 46820-R1, is hereby amended to add the Spanish language alerts services as outlined in the attached Exhibit A, which is incorporated herein as if copied in full, for a period from February 15, 2017, through April 5, 2017, at the prorated amount of $8,198.00. 3. The Agreement is hereby renewed for an additional Renewal Term effective from April 6, 2017, through April 5, 2018, to provide continued services for Accessible Hazard Alert System AHAS and the additional Spanish language alerts as outlined in the attached Exhibit A, at the annual rate of$63,194.00. Section 3 of the Agreement is hereby modified to increase the not to exceed amount from $44,944.00 to $65,000.00 annually. 5. All other terms, provisions, conditions, covenants and recitals of said Agreement not expressly amended herein shall remain in full force and effect. Name of Employee xi Title Exhibit A a& Exhibit A DEAF LINK Accessible Hazard Alert System (AHASTM) Program quote for City of Fort Worth AHAS Spanish language alerts Quote date: 1-18-2017 Proposal#1 Deaf Link will add AHAS Spanish language alerts to the existing Accessible Hazard Alert System(AHASTM) program for the City of Fort Worth. Emergency alerts will be sent in Spanish voice and text. The performance period is for February 15"2017 through April 5th,2017 and will coincide with the current ASL based AHASTM program which runs from April 6st 2016 thru April 5"2017. Scope of work:AHAS Spanish language alert program includes the following elements. 1. Deaf Link will create alerts in Spanish voice (audio file) and text based translations at the direction of the Fort Worth Office of EmergencManagement.Deaf Link will independently monitor the National Weather Service for Fort Worth and send Spanish language translations to subscribers registered in the AHASTM Spanish program database. Deaf Link will provide 24/7 capability to send AHASTM Spanish alerts in response to all Emergency Alert System(EAS) event codes for Warnings,Watches,and Emergencies, Civil Emergency Messages (CEM),Terrorism events,Amber Alerts, and National Weather Service(NWS)watches and warnings. 3. Deaf Link will modify the FTW AHASTM subscriber registration page and include a selection for Spanish language alerts. 4. AHASTM Spanish alerts will be sent in email and SMS formats. SMS messages will have the alert title with associated URL link embedded in the text. This service is free to the subscriber other than any carrier costs associated with their broadband(phone or internet) service provider. Scope of Work: 1. Deaf Link will provide 24/7 capability to send alerts in response to all Emergency Alert System(EAS) event codes for Warnings, Watches, and Emergencies,Civil Emergency Messages (CEM), Terrorism events,Amber Alerts, and National Weather Service(NWS)watches and warnings. Deaf Link will independently monitor the National Weather Service for Fort Worth. 2. Deaf Link AHASTM will send accessible web based alerts in American Sign Language (ASL)video, with English Voice,and Text(maximum length 2 minutes - 150 words). Note:ASL video alerts are created in Broadcast quality formats for possible use by local TV stations. 3. AHASTM Spanish alerts will be created in voice(audio file)and text translations and sent to subscribers. 4. AHASTM alerts include Short Message Service (SMS) capability for delivery of text based alerts to cell phones,web, or other mobile communication systems.Alert messages will have alert title with associated video link embedded in the text. 5. Deaf Link will host the FTW AHASTM website providing Fort Worth with a web location where accessible information about AHAS,Active Alerts, FAQ's and Preparedness videos in ASL,English voice/text can be viewed by the community. 6. Community outreach: Deaf Link can work with the City of Fort Worth in hosting community meetings to encourage personal preparedness and enrollment in the AHAS program.Meeting locations are determined and paid for by the City,broadband access for demonstrating AHAS is recommended. Meetings are a scheduled maximum of 2 hours in length. Deaf Link will charge$1500 per meeting which includes travel and provision of ASL interpreters. 7. The ShelterLinkTM program provides video based American Sign Language interpreting over the internet as well as Phone based interpreting(130 Spoken Languages)to support interpreting needs at Shelters. Costs for Shelterl-ink services are only incurred when FTW OEM requests activation of the program from Deaf Link.
