FORT WORTH CITY SECRETARY CONTRACT NO. 47215
PROFESSIONAL SERVICES AGREEMENT
This First Renewal of Fort Worth City Secretary Contract No. 47215 is entered into
by and between the City of Fort Worth (hereafter "City"), acting by and through Valerie R.
Washington, its duly authorized Assistant City Manager, and Price, Proctor & Associates,
LLP, ("Consultant"), acting by and through Dr. J. Randall Price and Dr. Timothy J. Proctor, its
duly authorized representatives.
WHEREAS, City and Consultant entered into a Professional Service Agreement for Pre-
Employment Psychological Services identified as Fort Worth City Secretary Contract No. 47215
(the"Agreement"), for a period beginning October 1, 2015 and ending September 30, 2016, with
four options to renew at the City's option; and
WHEREAS,the original term of the Contract expired on September 30,2016; and
WHEREAS,the City and Price, Proctor & Associates, LLP now wish to renew the term
of the Agreement for one year to end on September 30, 2017; and
NOW, THEREFORE, City and Consultant, acting herein by the through their duly
authorized representatives, enter into the following agreement to renew the contract:
1.
City and Consultant hereby agree to renew and extend Fort Worth City Secretary
Contract 47215 for a Renewal Term of one (1) year, beginning October 1, 2016, and ending
September 30,2017.
2.
All other provisions of the Contract which are not expressly amended herein shall remain
in full force and effect.
