Dear Ms.Oliver,
Your existing REVERSE 911®Support Services Agreement is set to renew December 1,2003.
This agreement covers the period of December 1,2003 through November 3042004. The
amount for each quarterly billing is$2027.50. We will continue to keep you on quarterly billing
per your request. The terms and conditions of your original License and/or Support Agreement
continue to apply.
To assure that we are in agreement in regards to this twelve month agreement based on quarterly
billing, lease an turn a copy of this letter to me.

The Sigma Communications,Inc.REVERSE 911® Support Services Agreement plan continues
to provide comprehensive support services:
. Professional technical support,provided by our experienced support staff
. A Database Update to keep your system up to date with your area's most current
phone numbers
. REVERSE 9119 Software Version Updates. based on your current operating platform
• Hardware Maintenance to keep your system in good repair and operational condition
The total price for City of Fort Worth's twelve month Support Agreement is$8,110.00. This
includes the new database and map update. Please issue payment to Sigma Communications,
Inc., SIGM 10062,PO Box 6200, Indianapolis,IN 46206-6200.
