Burleson & Associates, Inc. , (Engineer) made and entered into City Secretary Contract No. 27585, (the Contract) which was authorized by the City Council by M&C C-19901 on the 19th day of March 2002, and subsequently revised by three previous amendments; and WHEREAS, the Contract involves engineering services for the following project: Water and Sanitary Sewer Replacement Contract 2001 WSM-N WHEREAS, it has became necessary to execute Amendment No. 4 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated August 12, 2004, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $8,949.33. 2. Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $144,397.20. t ORIGINAL 3. (M/WBE Firm) will continue services for additional survey work on this project. As requested in your Uxe.d letter dated August 11. 2004 we will perfonn a design survey of the area within the right-of-way of Lenon:Drive between Virginia Place and Dorothy Lane, a distance of approximately 820 linear feet. We will also measure the depths of any associated manlholes if we are able to open-them. This survey will also show all topographic features and grade breaks located within the above-described areas. An electronic AutoCAD drawing will be provided containing contours at 1-foot intervals. A .dat, An, and ASCII points file will also be provided along with the Survey. DAL-TECH recommends a lurrip sum budget of$2,400,00 to complete these surveying services. If you are in agreement with this fee proposal, please sign in the space provided below and return this acknowledgement to our office. We will deliver the survey within 15 working days of your authorization to proceed. This will serve as our authorization to proceed and our right to enter the property for surveying purposes.
