COUNTY OF TARRANT $
WHEREAS, the City of Fort Worth (City) and Lockwood, Andrews &
Newnam, Inc. , (Engineer) made and entered into City Secretary
Contract No. 28790, (the Contract) which was authorized by the City
Council by M&C C-19585; on the 13th day of May 2003; and
subsequently revised by four previous amendments; and
WHEREAS, the Contract involves engineering services for the
following project:
Sanitary Sewer Rehabilitation Contract LXVII (67) , Part I
WHEREAS, it has become necessary to execute Amendment No. 5 to
said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:
1.
Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated February
10, 2005, a copy of which is attached hereto and incorporated
herein. The cost to City for the additional design services to be
performed by Engineer total $11,640.00.00.
2.
Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $410,716.00.

All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Survey and Engineering Design Services for Water Line Replacements
Sanitary Sewer Rehabilitation Contract LXVII (67)— Part 1

Dear Mr. Conlon:
Per your request, this Amendment No. 5 has been prepared for the performance of additional
survey and engineering design services with respect to the above referenced project. The work
specifically relates to the provision of water line replacements in the area of Harold and West 6th
Streets in the Part 1 portion of the project. Replacement of these water lines has been determined
necessary by the Water Department due to their old age and chronic related maintenance
requirements.
The additional work will require the survey and engineering design of approximately 1,000
linear feet of new water line projected to be of 8-inches in diameter. This is in addition to the
survey and design work previously completed under the original contract scope. A revised
preliminary opinion of construction costs for the new water lines is $120,000.
Based upon the work to be performed as described above, a lump sum fee addition of$11,640 is
hereby requested. A summary of the contract values affected by the amendments including this
Amendment No. 5 as proposed herein follows at the bottom of this letter. This fee includes
$1,650 that will be performed by our M/WBE sub-consultant. This amount translates to 14.18%
of the amendment.
