IN ACCORDANCE WITH THIS CHANGE ORDER,THE CONTRACTOR SHALL: modify/change the contract as shown below to provide a complete,functional,and
operational elevated storage tank meeting current regulatory requirements.
This change order consists of a time extension request from the Contractor,as shown in the attached letter,dated March 7,2017.
This construction contract required substantial completion within 120 calendar days from the date of the Notice to Proceed. Tile date of the Notice to Proceed was October
17,2016. The date substantial completion was achieved was March 8,2017,a duration of 142 calendar days.
The Contractor has requested an additional 14 calendar days for roof support beam repairs and 27 calendar days for weather delays,resulting in a total time extension request
of 41 calendar days.
Staff has reviewed this change order and is in agreement with time extension request. Documentation providing additional information is attached to and made part of this
change order. This change order would extend the contract time to allow 161 calendar days for substantial completion,and 191 calendar days for final completion.
It is understood and agreed that the acceptance of this Change Order by the Contractor constitutes an accord and satisfaction and represents payment in full
(both in time and money)for all costs arising out of,or incidental to,the Change Order.
