WHEREAS, the City of Fort Worth (City) and Teague Nall and
Perkins, Inc. , (Engineer) made and entered into City Secretary
Contract No. 27917, (the Contract) which was authorized by the City
Council by M&C C-19183; on the 8th day of August 2002 and
subsequently revised by one previous amendment; and
WHEREAS, the Contract involves engineering services for the
following project:
Water Replacement Contract 2002 STM-C, F and G.
WHEREAS, it has become necessary to execute Amendment No. 2 to
said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:
1.
Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated April 5,
2005, a copy of which is attached hereto and incorporated herein.
The cost to City for the additional design services to be performed
by Engineer total $3,922.00.
2.
Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $203,376.00.

3.
All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Fee Proposal for Contract Amendment No. 2
Dear Mr. Sholola:
Teague Nall and Perkins, Inc. is submitting this fee proposal for Amendment No. 2 to the contract
for the above referenced project based on additional services requested. The City is re-bidding
STM-F portion of the water replacement project. The following is a brief description and cost for the
additional services to be provided.:
1. Additional Services for Printing Plans and Specifications
The additional compensation for printing 47 sets of the final approved plans and 40 sets of
the final approved specification documents for re-bidding purposes is summarized below:
• Forty (40) sets of final approved plans will be printed
for re-bidding. Seven (7) sets of plans will be printed
for distribution to City staff = $ 1,222
• Forty (40) sets of final approved specification
documents will be printed for re-bidding = $ 1,200
Subtotal for Printing Plans and Specifications = $ 2,422
2. Additional Services for Bidding Assistance and Recommendation of Award
Services for providing bidding assistance and recommendation of Award include:
• Preparation of the tabulation and review of all bids received,
• Attending the bid opening,
• Providing the City with a recommendation of award.
Subtotal for Bidding Assistance and
Recommendation of Award = $ 1,500

For additional services provided pursuant to this Amendment No. 2,we request compensation in an
amount not to exceed $3,922.00. Such payment shall be made in accordance with the payment
terms specified in the Contract.
Summary of Contract Fee:
Original Contract Amount: $188,216.00
Amendment No. 1 (Not-to-Exceed): $ 11,238.00
Amendment No. 2 (Not-to-Exceed): $ 3,922.00
Revised Contract Amount (Not-to-Exceed): $203,376.00
