WHEREAS, the City of Fort Worth (City) and Teague Nall & Perkins
Inc, (Engineer) made and entered into City Secretary Contract No. 33083,
(the Contract) which was authorized by the City Council by M&C C-21178 on
the 6th day of December 2005 .
WHEREAS, the Contract involves engineering services for the
following project:
Engineering Design Services: Street Reconstruction, on Chapel Creek
Boulevard ( Chapin rd to I - 30 ) , Project No. 00139 .
WHEREAS, it has become necessary to execute Amendment No. 1 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their
duly authorized representatives, enter into the following agreement which
amends the Contract:

Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated May 1 ,2006, a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer total
$ 13, 615.00 .

Article II of the Contract is amended to provide for an increase in
the maximum fee to be paid to Engineer for all work and services
performed under the Contract, as amended, so that the total fee paid by

the City for all work and services shall not exceed the sum of
$ 74, 913 .00 .
3 .
All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Fee Proposal for Contract Amendment No. 1

Dear Mr. Jaafari:
Teague Nall and Perkins, Inc. is submitting this fee Amendment to the above referenced contract
based on additional services requested. The following is a brief description and cost for the
additional services provided on this project:
Additional Services to prepare ROW Acquisition Documents
Re-alignment of Chapel creek requires acquisition of Right-of-Way along the east side of the
roadway. Initially, the need for ROW was uncertain based on a review of TAD maps.
However, survey field data combined with deed research indicates a need for the acquisition
of additional ROW. The following fees include applicable field survey, preparation of metes
and bounds, exhibits, and setting iron rods at property corners.
■ Chapel Creek ROW Documents 5 @ $1,350/ea. _ $ 6,750
Additional Services to prepare Easement Documents
Temporary Construction Easement parcels are required to perform grading work outside of
the proposed ROW along the east side of the roadway and a small portion along the west
ROW just south of Chapin Road. The following fees include preparation of easement
exhibits.
■ Temporary Construction Easements 5 @ $375/ea. _ $ 1,875
Additional Services to perform Geotechnical Testing and Report
A geotechnical report is required to determine the pavement thickness and base design.
The work includes borings,soil evaluation,paving recommendations,and the preparation of
a report (3-copies will be provided to the City).
