THE CITY OF FORT WORTH AND SENIOR CITIZENS SERVICES OF GREATER WARRANT COUNTY INC. On ("Effective Date"), this ADDENDUM is made and entered by and between the CITY OF FORT WORTH, a home rule municipal corporation of the State of Texas(hereinafter referred to as "City") acting by and through its duly authorized Assistant City Manager, and Senior Citizens Services of Greater Tarrant County, Inc. (hereinafter referred to as "SCS") acting by and through it's duly authorized Executive Director. WHEREAS, on August 10, 2016, the City and SCS executed a 6-year license agreement known as City Secretary Contract ("CSG") No. 48074 ("Agreement") to license various City Community Centers and Fellowship Corner Senior Center to provide programs and services to benefit seniors living in the vicinity of the centers. The license agreement term commencement date is April 1, 2016, and terminates on March 31, 2021, NOW, THEREFORE, City and SCS hereby mutually agree that the Agreement is hereby amended to add the addendum to the Agreement as follows: Add the following to Section 4(E) of the Agreement: Membership Cards: Every participant in the SCS i program must have a membership card to use the City vehicles and to participate in any programs outside of the SCS sponsored program hours. Replace Section 3 of Exhibit B with the following: 3. Paper Supplies. SCS will provide all the paper goods for the meal program, which includes items such as paper plates, plastic ware, gloves, aprons, hair nets, etc. SCS will help provide toiletry items as needed at the Community Centers being used for the program. SCS is exclusively responsible for all paper goods, toiletry and supplies for Fellowship Corner until Fellowship Corner moves to another city facility. j I Add the following sections to Exhlbit B: i 11) Protocol for reporting Accidents/incidents: SCS will report all accidents/incidents to City Staff and complete all required paperwork within 48 hours of incident. i 12) Communication: SCS will invite Center Coordinators/Supervisors periodically to attend SCS Center Council meetings and include all Center Coordinators/Supervisors on the email correspondence list for that meeting. i All of the terms and provisions of the Agreement between City and SCS shall remain in full force and effect except as specifically amended herein through this addendum.
