THIRD EXTENSION TO CITY SECRETARY
CONTRACT NO 33157
THIS THIRD EXTENSION to City Secretary Contract No. 33157 (hereinafter "Extension") is
entered into by and between the City of Fort Worth("City"),a home-rule municipality in the State of
Texas,acting through Fernando Costa ,its duly authorized Assistant City Manager,and
Heritage Environmental Services,LLC("Contractor"),an Oklahoma corporation duly authorized to
do business in Texas by and through Dean Croft, its duly authorized Vice-President.
WHEREAS,the City and Contractor entered into that certain agreement identified as City Secretary
Contract No. 33157 (also the "Contract") commencing on January 23, 2006 and ending December
31, 2006 with three one-year extensions for the provision of Transportation and Disposal of
Household Hazardous Waste for the Environmental Collection Center; and
WHEREAS, the City and Contractor wish to exercise the third extension option;
WHEREAS,the parties agree that all other conditions,terms and pricing shall remain the same as
stated in City Secretary Contract No. 33157.
NOW,THEREFORE,for and in consideration of the mutual covenants contained herein,and other
good and valuable consideration,the receipt and sufficiency of which are hereby acknowledged and
confessed, the parties hereto agree as follows:
The Agreement known as City Secretary Contract No 33157 shall be extended pursuant to Section 4
"Term"for its third one(1)year extension beginning January 1,2009 through December 31,2009.
