AMENDMENT TO CITY SECRETARY CONTRACT NO. 31986 WHEREAS, on June 23, 2005, the City of Fort Worth ("City") and Bridge Youth and Family Services ("Contractor") made and entered into City Secretary Contract No. 31986 (the "Contract"); WHEREAS, the Contract provided funding to Contractor pursuant to a grant received by City from the United States Department of Housing and Urban Development ("HUD") through the Community Development Block Grant ("CDBG") Program No. B-05-MC-48-0010, Catalog of Federal Domestic Assistance ("CFDA") No. 14.218, to provide services to the citizens of the City; WHEREAS, the assigned Catalog of Federal Domestic Assistance (CFDA) number is 14.218 for this Federal program; WHEREAS, the Contract was entered into so that Contractor would administer youth services to the citizens of the City under the program name Buffalo Soldiers; WHEREAS, effective October 1, 2005, Contractor entered into an Administrative Services Agreement with All Church Home for Children ("All Church"), a non-profit organization, to perform all of Contractor's administrative functions on behalf of Contractor; and WHEREAS, it the mutual desire of City and Contractor to amend City Secretary Contract No. 31986 to reference the Administrative Services Agreement with All Church and to acknowledge All Church's legal authorization to act on behalf of Contractor. NOW, THEREFORE, City, whose address is 1000 Throckmorton St., Fort Worth TX 76102, acting by and through Dale Fisseler, its duly authorized Assistant City Manager, and Contractor, whose address is 115 West Broadway, Fort Worth, TX 76104, acting by and through Wayne Carson, Ph.D., Chief Executive Officer of All Church, Contractor's duly authorized administrative services provider, do hereby agree as follows: I. The first paragraph of the Contract is amended to be read as follows: "THIS contract ("Contract") is made and entered into by and between the City of Fort Worth ("City"), whose address is 1000 Throckmorton St., Fort Worth Texas 76102, acting by and through Dale Fisseler, its duly authorized Assistant City Manager, and Bridge Youth and Family Services ("Contractor"), whose address is 115 West Broadway, Fort Worth, Texas 76104, acting by and through Wayne Carson, Ph.D., Chief Executive Officer of All Church Home for Children ("All Church"), Contractor's duly authorized administrative services provider pursuant to an Administrative Services Agreement by and between All Church and Contractor effective October 1, 2005). (Sometimes City and Contractor are referred to individually as a "Party" and collectively as the "Parties".)" Housing Opportunities for Persons with HIV/AIDS (HOPWA) Organization Amount AIDS Outreach Center $460,970.00 Tarrant County Samaritan Housing, Inc. $240,889.00 AIDS Outreach Rural Texas $103,141.00 TOTAL HOPWA $805,000.00 DISCUSSION: On April 5, 2005, the City Council adopted the budgets and the appropriation ordinance increasing estimated receipts and appropriations for the Year XXXI (June 1, 2005 to May 31, 2006) Community Development Block Grant (CDBG) grant funds, (2005-2006) Emergency Shelter Grant (ESG) grant funds and (2005-2006) Housing Opportunities for Persons with AIDS (HOPWA) grant funds as part of the City's http://www.cfwnet.org/council_packet/Reports/mc_print.asp 3/8/2006 Page 3 of 3 Consolidated Plan and Consolidated Action Plan. The Consolidated Plan, as adopted by the City Council, included program descriptions on each program listed above. The above programs are consistent with the City's approved funding policies and priorities. Each program meets the requirements of the Department of Housing and Urban Development (HUD) regulations defining "eligible activity." These programs prove services citywide. FISCAL INFORMATION/CERTIFICATION: The Finance Director certifies that funds will be available in the current operating budget as appropriated, in the Grants Fund contingent upon approval and receipt of adequate funding from the U.S. Departmental of Housing and Urban Development (HUD) for the term beginning June 1, 2005, and ending May 31, 2006.
