TO CITY SECRETARY CONTRACT NO. 30880
CONTRACT FOR PROFESSIONAL SERVICES
This Amendment to C ntract for P fe si n��^I Services (the "Amendment") is
made and entered into this day of� � by and between the City of
Fort Worth ("City"), a home-rule municipal corporation situated in Tarrant, Denton and
Wise counties, Texas, acting by and through its duly authorized Assistant City Manager,
and Concentra Health Services, Inc., a Nevada corporation, d/b/a/ Concentra Medical
Centers ("Concentra").
WITNESSETH:
WHEREAS, City and Concentra have entered into a certain Contract for
Professional Services, ("the Agreement") effective as of November 9, 2004 ("Effective
Date"), same being City of Fort Worth City Secretary Contract No. 30880; and
NOW, THEREFORE, in consideration of the foregoing and mutual covenants and
agreements contained herein and for other good and valuable consideration, the receipt
and sufficiency of which are hereby acknowledged, City and Concentra hereby agree to
amend the Agreement as follows:
Section 1.
Appendix "B" — Schedule for Compensation, attached to City Secretary Contract
No. 30880 is hereby amended to include the following Services and Pricing Units:

networks at which time the discount rate may be negotiated directly between the City
or its third party administrator and Concentra
Year One and Year Two costs as set forth in this amendment include any and all
injury care fees due to the City. The City's third party administrator passes on a 2%
discount to the City, while Concentra discounts the remaining percentage to arrive at
the negotiated rate as stated above.

are not amended herein continue in full force and effect.

Authorization to Renew the Agreement with Concentra Medical Centers as the City's Preferred
Occupational Health Provider
RECOMMENDATION:
It is recommended that the City Council authorize the City Manager to exercise the first of four one year
options to renew the agreement with Concentra Medical Centers (Concentra) beginning October 1, 2005
and expiring September 30, 2006.
DISCUSSION:
On September 14, 2004 (M&C C-20266) the City Council authorized the City Manager to execute a contract
with Concentra Medical Centers to provide occupational health care services for the City of Fort Worth. The
City's current contract with Concentra which began October 1, 2004 and expires September 30, 2005,
includes four one-year options for renewal.
Concentra is one of the largest occupational health care providers in the nation. There are three clinics in
Fort Worth and 14 clinics in the Metroplex. A majority of the services provided by Concentra under this
contract include: medical treatment for on-the-job injuries, drug and alcohol testing and counseling, pre-
employment physicals and vaccinations.
Concentra's services are priced on a per unit basis and the medical treatment for on the job injuries is
discounted 17 percent lower than the Texas Workers' Compensation fee guideline. It is anticipated that the
cost for all services during FY 2005-06 will be approximately $617,651.00.
M/WBE - Concentra is in compliance with the City's M/WBE Ordinance by committing to 10% M/WBE
participation medical services for occupational injuries. The City's goal on this project is 10%.
FISCAL INFORMATION/CERTIFICATION:
The Finance Director certifies that funds are available, as appropriated in the FY 2005-06 General and
Workers' Compensation Funds adopted Budget.
