CITY SECRETARY CONTRACT NO. 48516
PROFESSIONAL SERVICES AGREEMENT
This Amendment is entered into by and between the City of Fort Worth (hereafter
"City"), a home rule municipal corporation in Texas, with its principal place of business at 200
Texas Street, Fort Worth, Texas, and B1ackLine Systems, Inc., (hereafter"Contractor").
WHEREAS, the parties have previously entered into City of Fort Worth City Secretary
Contract No. 48516 (the "Agreement"), which was executed on December 29, 2016;
WHEREAS,the Agreement involves professional services for the purpose of task
management support for the hosted service online support portal and 25 licenses; and
WHEREAS, City and Contractor wish to amend the Agreement to increase the number
of licenses by 45, for a new total of 70 licenses, at a cost of$11,655.00, with the additional 45
licenses effective through December 28,2016 to be coterminous with the original licenses;
NOW, THEREFORE, City and Contractor acting herein by the through their duly
authorized representatives, enter into the following Amendment to the Agreement:
1.
Exhibit B. Exhibit B is hereby amended to add an additional 45 task management
licenses, for a total of 70 licenses. The annual cost for all 70 licenses is $31,080.00, paid
annually, and invoiced in December of each year of the contract.
2.
All other provisions of the Agreement not expressly amended herein shall remain in full
force and effect.
