Home My WebLink AboutContract 43534-A4 AMENDMENT No.4 CITY SECRETARY t� J? ` q y CONTRACT NO._ `7 a TO CITY SECRETARY CONTRACT < 9S b £ WHEREAS, the City of Fort Worth (CITY) and Criado & Associates, Inc. , (ENGINEER) made and entered into City Secretary Contract No. 43534, (the CONTRACT) which was authorized by M&C C-25758 on the 24th day of August, 2012 in the amount of $184,347 . 00; and WHER +LAS, the CONTRACT was subsequently revised by: Amendment Number 1 in the amount of $47 , 460 .00 which was administratively authorized on October 2 , 2013, Amendment Number 2 in the amount of $5, 000.00 which was Administratively authorized on April 7, 2014 and Amendment No. 3 in the amount of $49,500 . 00 which was administratively authorized on September 28, 2015; and WHEREAS, the CONTRACT involves engineering services for the following project: Water and Sanitary Sewer Replacement Contract 2011, WSM-A; and WHEREAS, it has become necessary to execute Amendment No. 4 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated October 3, 2017 , a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $47,188. 00 (water: $28, 188 . 00; sewer: $19,000 .00) . 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services s1 a bean amount of $333,495.00. (CRIADO) is pleased to s u b m i t Amendment No.4 t o C i t y S e c r e t a r y Contract No.43534for additional Engineering Services for the project "Water and Sanitary Sewer Replacement Contract 2011,WSM-A'. This amendment includes the following task: PROJECT MANAGEMENT CRIADO's contract schedule will be extended to December 30,2018. FINAL DESIGN CRIADO will provide three separate construction packages, instead of one, and incorporate up to three sets of designs "by others" into the first construction package. CRIADO will provide final design plans and estimates of probable construction cost, using standard CITY bid items for each part. CRiADO will provide construction documents and project manuals for each part. BID PHASE AND CONSTRUCTION PHASE SERVICES CRIADO will attend pre-bid meetings, public meetings, and pre-construction meetings. CRIADO will upload documents related to this amendment to Buzzsaw and maintain the plan holders list. CRIADO will prepare three mylar cover sheets and will attend the bid opening and assist the CITY with tabulating the bids. CRIADO will make an award recommendation to the CITY based on the documents submitted by the bidders. Our proposal to perform the additional engineering services is $47,188.0o and with approval of this amendment,the total fee for all work and services will be$335,485.00.
