S - TO CITY SECRETARY CONTRACT WHEREAS, the City of Fort Worth (CITY) and Gresham, Smith & Partners, (ENGINEER) made and entered into City Secretary Contract No. 46599, (the CONTRACT) which was authorized by M&C C-27231 on the 17th day of March, 2015 in the amount of $206,325.00; and WHEREAS, the CONTRACT involves engineering services for the following project: Sanitary Sewer Rehabilitation, Contract 89; and WHEREAS, it has become necessary to execute Amendment No.l to the CONTRACT to include an increased scope of work and revised fee. NOWTHEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated January 9, 2018, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $8,640 .00. 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $214,965 .00 3 . Senior Professional Engineer City of Fort Worth—Water Department 200 Texas Street Fort Worth,TX 76102 Subject: City of Fort Worth Project No. 02381,Sanitary Sewer Rehabilitation, Design Contract 89,Amendment No.1 Walter Herewith is our request for Amendment No.1 for the subject project. The Amendment includes additional scope and fee to prepare drawings for an additional line, Segment L-2421. The line is located.between Park Ridge Boulevard and Mission Street. The line will be rehabilitated or replaced from the manhole (Station 4+30)just west of Lubbock Avenue to the upstream end of the line, approximately 700 lineal feet.The additional scope and fee is as follows: Task No.1 Project Management • Manage the design team, provide quality assurance,coordinate internal meetings, and prepare timely invoicing. Task No.2 Concept Design • Visit the site, collect available data, and prepare base sheets using available aerial base mapping and layout line on plan. Task No.3 Preliminary Design • Prepare preliminary plan and profile sheets and submit to the City for review. Task No.4 Preliminary Final Design • Incorporate City review comments on Preliminary Plans and prepare 90%complete plan and profile sheets and submit for City review. • Finalize 100% plan and profile sheets and submit to City to issue to contractor. Fee Attached, please find the Level of Effort Spreadsheet showing our fee breakdown.We propose to perform this work for a lump sum fee of$8,640.00. The City will provide field survey limited to top of rim and invert elevations of the two existing manholes on Segment L-2421.The city will assist the contractor in obtaining access to the site and permission to construct the proposed improvements.
