Professional Services Agreement between the City of Fort Worth ("City") and
PsychScreening
Contract No. CSC No. 46676 Al (the "Contract')
Renewal Term No. 3: October 1, 2017 to September 30, 2018
The above referenced Contract expired on September 30, 2017. However, pursuant to the Contract,
renewals are at the mutual agreement of the parties. This letter is to confirm the Parties renewal of CSC
No. 46676, as amended, for an additional one year period, which began on October 1, 2017, and will end
on September 30, 2018. All other terms and conditions of CSC No. 46676, as amended, remain
unchanged.
Please sign and return three (3) signed copies of agreement letter, along with a copy of your current
insurance certificate, to the address set forth below.
