This Supplement is to be attached to and become part of that certain DOCUmanage Agreement entered into by and
between DOCUmation of North Texas, Inc., now known as UBEO of North Texas, Inc. ("Previous Owner" or
"DOCUmation"), and the City of Fort Worth (the "Customer" or "City"), collectively the "parties," entered into on or about
December 24, 2013, as amended and supplemented by Contract No. 45231, Supplement 1 entered into on or about
March 19, 2014 and 45231, Supplement 2, Revision 1 entered into on or about April 23, 2014, all as amended and
supplemented by that certain Addendum effective March 10, 2016 (the"2016 Addendum") and that certain Amendment to
Agreement dated July 27, 2016 (collectively and as further amended and supplemented from time to to time, collectively
the "Agreement'). The Agreement is on file with, and referred to for convenience of reference only by the City Secretary
as CSC No. 45231, as amended and supplemented by CSC No. 45231-S1 and CSC No. 45231-S2, and shall not affect
the meaning, construction or effect of the provisions hereof or of the Agreement. The Agreement has been assigned by
Previous Owner to U.S. Bank Eqiupment Finance, a division of U.S. Bank National Association ("Assignee" or "Owner").
The Agreement is on file with, and referred to for convenience of reference only by Assignee as Agreement No. 500-
0386202-003, and shall not affect the meaning, construction or effect of the provisions hereof or of the Agreement. The
words you and your refer to Customer. The words we, us and our refer to Owner. If any terms hereof are inconsistent with
the terms of the Agreement, the terms hereof shall prevail.
The parties wish to amend and supplement the above-referenced Agreement as follows:set forth below in the box marked
with an "X":
X Exhibit "A," Equipment List, to the 2016 Addendum is amended by equipment additions and deletions
described in Exhibit "A" hereto., which Exhibit A hereto also includes all of the equipment subject to the
2016 Addendum
X_ Exhibit "B," Payment Schedule, to the 2016 Addendum is amended in its entirety and replaced with
Exhbit"B" hereto.
X Exhibit "C," Equipment Fees, to the 2016 Addednum is amended in its entirety and replaced with
Exhbit"C" hereto.
Except as amended herein, all other terms of the Agreement remain in full force and effect.
