WHEREAS, the City of Fort Worth (City) and Garcia & Associates
Engineering Inc. (The "Engineer") made and entered into City Secretary
Contract No. 24883, (the Contract) which was authorized by the City
Council by M&C C-17334 on the 23 day of March , 1999; and
WHEREAS, the Contract involves engineering services for the following
project:
Water and sanitary sewer line replacement in Rosedale Street, Contract
No. 1, S. Main St. to I.H. 35W.
WHEREAS, it has become necessary to execute Amendment No.3 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their
duly authorized representatives, enter into the following agreement which
amends the Contract:
1.
Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated August 3, 2001 a
copy of which is attached hereto and incorporated herein. The cost to City
for the additional services to be performed by Engineer total $3, 660. .00
2.
Article II of the Contract is amended to provide for an increase in
the maximum fee to be paid to Engineer for all work and services performed
under the Contract, as amended, so that the total fee paid by the City for
all work and services shall not exceed the sum of $467, 163.00

All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Improvements Around Tarrant County Public Health Facility
Dear Mr. Ehia:
On behalf of Garcia and Associates Engineering, Inc. (G&A), I am pleased to present the
following proposal to provide engineering design services for the above referenced site. These
design services are not included in the original scope of services and thus require additional
authorization. It has become necessary to execute this change to the above referenced contract
to include an increased scope of work and revised maximum fee.
Additional Scope of Services
Sanitary Sewer
• Cut and plug existing six inch sanitary sewer
• Provide eight inch stub-out for line L-2047
• Abandon remaining line and two manholes for line L-2047
• Provide eight inch stub-out for line L-273
Water
• Provide 2 two inch service connections on S. Main
• Provide 1 eight inch fire line extension and hydrant on S. Main
• Provide 2 two inch service connections on Bryan Avenue
• Remove two fire hydrants on Bryan Avenue
• Remove and salvage two 6 inch gate valves on Bryan Avenue
• Cut and plug existing 6 inch water line on Bryan and Magnolia
• Remove and salvage two 4 inch gate valves on Crawford Street
• Remove and salvage one 8 inch gate valve on Crawford Street
• Cut and plug existing eight inch water line at Crawford and Magnolia
