FIRST RENEWAL OF CITY SECRETARY CONTRACT NO.46329
PROFESSIONAL SERVICES AGREEMENT

This First Renewal of City Secretary Contract No. 46329 — Professional Services Agreement ("First
Renewal') is made and entered into this �ti day of rc),, , 201% by and between the City
of Fort Worth ("CITY'), a home-rule municipal corporation of the State of Texas, acting by and through its
duly authorized Assistant City Manager, and Occupational Health Centers of the Southwest, P.A., DBA
Concentra Medical Centers ("PROVIDER"), a Texas Professional Association, acting by and through its
duly authorized Senior Vice President and Chief Medical Officer.
WITNESSETH:
WHEREAS, CITY and PROVIDER entered into a Professional Services Agreement effective
January 1, 2015,the same being Fort Worth City Secretary Contract No.46329(the"Agreement'), and
WHEREAS, pursuant to the Agreement, PROVIDER provides occupational injury and non-injury
occupational health care services to CITY employees; and
WHEREAS, the parties previously amended the Agreement, that amendment being Fort Worth
City Secretary Contract No. 46329-A1; and
WHEREAS, the Agreement provides for an initial, three-year term and two, one-year renewal
options; and
WHEREAS, CITY and PROVIDER wish to extend the Agreement, as amended, from January 1,
2018 through December 31, 2018.
NOW THEREFORE, in consideration of the foregoing and the mutual covenants and agreements
contained herein, CITY and PROVIDER hereby agree as follows:
1. CITY and PROVIDER hereby agree to renew and extend Fort Worth City Secretary Contract No.
46329, as amended by Fort Worth City Secretary Contract No. 46329-A1, for a one-year renewal
period, beginning January 1, 2018 and ending December 31, 2018, under the same terms and
conditions unless otherwise further amended herein.

2. All other terms and conditions of Fort Worth City Secretary Contract No. 46329, as amended by
Fort Worth City Secretary Contract 46329-A1, that are not amended herein shall remain in full force
and effect throughout the renewal period.
