FIRST RENEWAL AND AMENDMENT NO. 1 TO
CITY SECRETARY CONTRACT NO. 48873
PROFESSIONAL SERVICES AGREEMENT
This Amendment and Renewal is entered into by and between the City of Fort Worth
(hereafter "City"), a home rule municipal corporation organized under the laws of the State of
Texas, with its principal place of business at 200 Texas Street, Fort Worth, Texas, and
Veterinary Health Care Concepts, PC dba Hulen Hills Animal Hospital (hereafter
"Veterinarian"), a Texas Corporation.
WHEREAS, the parties have previously entered into City of Fort Worth City Secretary
Contract No. 48873 (the "Contract"), which was executed on February 14, 2017; and
WHEREAS, the Contract provides veterinary services for Fort Worth Police Department
canines, which includes physical exams, x-rays vaccines, dentistry and other diagnostic testing to
screen for early illness and diseases; and
WHEREAS, the original term of the Contract expired on February 19, 2018; and
WHEREAS, the City and Veterinarian agreed to renew the Contract and continued
working under the Contract while this document was prepared; and
WHEREAS, the City and Veterinarian now wish to evidence the parties intention to
renew the Contract for its first renewal term; and
WHEREAS, the City and Veterinarian also wish to amend the original Contract to set
renewal term dates to expire on September 30th of each renewal year.
NOW, THEREFORE, City and Veterinarian, acting herein by the through their duly
authorized representatives, enter into the following agreement:

1. RENEWAL
The term of the Contract is hereby renewed beginning on February 20, 2018 and ending
on September 30, 2018. Each subsequent renewal shall be for a one-year period as better defined
in section 2 of this document.
2. AMENDMENT
Provision 2: TERM of the Contract shall be replaced in its entirety with the following:

This Agreement shall begin on February 20, 2017 ("Effective Date") and shall expire on
February 19, 2018 ("Expiration Date"), unless terminated earlier in accordance with this
Agreement ("Initial Term"). Following the Initial Term, there shall be four(4) options to renew
the City's sole discretion (each a "Renewal Term"). The first renewal term shall begin on
February 20, 2018 and end on September 30, 2018. Each renewal term thereafter shall have a
one-year term from October 1st —September 30th. The City shall provide Veterinarian with
written notice of its intent to renew at least thirty (30)days prior to the end of each term.
3.
All other provisions of the Contract which are not expressly amended herein shall remain
in full force and effect.
