DISCUSSION: The Sidewalk Improvements project includes the construction of approximately 10,400 linear feet of concrete sidewalk and twenty Americans with Disabilities Act barrier free ramps along Seminary Drive from Hemphill Street to Campus Drive and along Campus Drive from Mansfield Highway to Joe B Rushing Road. Construction is expected to begin in May 2018 and expected to be completed in December 2018. The project was advertised for bids in the Fort Worth Star-Telegram on January 11, 2018 and January 18, 2018. On Page 2 of 7 Section 1.2 A. 1. c. 5), Grade rings or other adjustment device; for AT&T and ATMOS manholes grade rings will be provided by franchise utilities (AT&T and ATMOS). Contractor will coordinate delivery, installation, and inspection of grade ring or other adjustment devices. 3. See Attached revised specification document. VI. Plan Sheets are revised as follows: 1. On Sheet 7 of 45 revise Gas (ATMOS) Manhole Adjustment call-out near STA. 10+35 to read as follows; "CONTRACTOR TO ADJUST GAS MH RIM TO MEET FINAL GRADE; COORDINATE WITH ATMOS FOR GRADE RINGS AND INSPECTION" 2. Provide details of any instances when the Contractor, any present partners or company officers have filed for bankruptcy or have been part of a company that has filed for bankruptcy. 5. Any Contractor who becomes qualified under these provisions and remains in good standing with the City will only be required to submit a newly compiled or reviewed financial statement before each 2 year anniversary of initial qualification. Should a Contractor fail to submit the required information before any 2 year anniversary, he/she will be so advised and will be allowed 6 months to provide the required financial statement.
