IN ACCORDANCE WITH THIS CHANGE ORDER,THE CONTRACTOR SHALL,
Change Order#f1 to City Secretary Contract No.49988 Contract with Cobb Environmental and Technical Services,Inc.for additional costs incurred during initial
phases of the UST removal and plan changes requested by the Water Department.The amounts are listed below for each individual change order item:
1. Over-excavation of UST tankhold;transportation and disposal of contaminated soils;delays in regards to potential changes from Water Department;
2. Repair of utility conduit damaged during UST removal;utility conduit was not marked on any documentation from Water Department and was not able
to be located during utility locate services;
3. Installation of subpanel to existing electrical panel in order to install neutral switch to operate new system;
4. Utilize flowable fill to abandon-in-place third UST due to inability to remove UST due to communication conduit running above UST location;
5. Placement of crushed stone over UST in location of utility conduits to serve as replacement of select fill and compaction;
6. Overexcavation of contaminated soils in and around tankhoid,transportation,and disposal of 249 cubic yards of petroleum impacted soils.
All work to be performed in accordance with the Proposal Specifications.
It Is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents in full(both time and
money)for all costs arising out of,or incidental to,the above Change Order.
