Your existing REVERSE 9110 Support Services Agreement is set to renew December 1,2004.
This agreement covers the period of December 1,2004 through November 30,2005. The
amount for each quarterly billing is $1,147.00. We will continue to keep you on quarterly billing
per your request. The terms and conditions of your original License and/or Support Agreement
continue to apply.
To assure that we are in agreement in regards to this twelve month agreement based on quarterly

provide comprehensive support services:
• Reverse 911 new releases and software updates
• Reverse 911 24X7 technical support
• Onsite hardware support
• Reverse 911 system loaner program
• Ongoing Reverse 911 training
• Reverse 911 newsletter providing tips and news from Reverse 911 users nationwide
• Discounts on future software and hardware purchases
The total price for City of Fort Worth's twelve month Support Agreement is$4,588.00. Please
issue payment to Sigma Communications, SIGM 10062,PO Box 6200, Indianapolis,IN 46206-
6200.
