FOR ARCHITECTURAL DESIGN SERVICES

WHEREAS, The City of Fort Worth and Perkins Will (Architect) made and entered into City Secretary
Contract No. 32411 (The Contract) which was authorized by the City Council by M&C C-20926, the 30th
day of August, 2005; and
WHEREAS, further amendment is necessary to reimburse the Architect for including the vacation of an
existing utility easement and dedication of a new utility easement within the 1100 Nashville Avenue platted
property.
NOW THEREFORE, City and Architect, acting herein by and through their duly authorized
representatives, enter into the following agreement which amends the Contract:
1.
Article 1.1, SECTION I —SERVICES OF THE ARCHITECT, a paragraph is added to read as follows:
"Provide additional design services to include the vacation of existing utility easement and the
dedication of a new utility easement within the platted property. The cost of this additional service
is $2,675.00 total. The scope of services is described more fully in the North Texas Surveying
letter dated January 10, 2011 reference: Utility Easement proposal on a portion of the Fort Worth
Police Station located at the Northwest corner of Avenue D and Nashville Avenue in the City of
Fort Worth, Tarrant County, Texas, which is attached hereto and as Attachment "A" and made a
part of this Agreement."

Paragraph 1 of SECTION V-COMPENSATION TO ARCHITECT, is amended as follows:
"The total compensation for all of the assignments to be performed by Architect as described in
CHARACTER AND EXTENT of ARCHITECT'S SERVICES hereof shall be a fixed fee of
$838,075.00 plus up to $56,948.31 in reimbursable expenses."

All other provisions of this Contract which are not expressly amended herein shall remain in full force and
effect.

Thank you for the opportunity to submit this proposal for the Topographic and Exhibit
Surveys at the above listed property.
SCOPE: Topographic survey on an area agreed upon in a meeting with Alfonso Meza,
Alan Bagamery,Michael Arthur and David Hutchinson on Wednesday,December 8.
2010 from 11:00 am— 12:00 pm at Police Station site.
DETAIL:
1. Contact DIGTESS,a utility locate service to mark underground utilities in
subject area.
2. Create drawing depicting existing utilities located by DIGTESS of property
boundary only.
3. Create easements and abandonment's with a survey exhibit and a legal
description.
Topographic Survey-$975.00
Easement Exhibit-$850.00
Easement Abandonment-$850.00
ASSUMPTIONS: NTS will have permission to access subject property.
