COUNTY OF TARRANT
WHEREAS, The City of Fort Worth (the "City') and Elements of Architecture, Inc. (the "Architect") made
and entered into City Secretary Contract No.45276(The Contract)which was administratively executed in
the amount of$27,220.00.
WHEREAS, the City administratively executed Amendment No. 1 on July 31, 2014 in the amount of
$22,373.00.
WHEREAS, the City executed Amendment No. 2 on October 27, 2015 by Council approval of M&C C-
27517 in the amount of$147,099.00.
WHEREAS, the City administratively executed Amendment No. 3 on March 1, 2017 in the amount of
$5,570.00.
WHEREAS, The City administratively executed Amendment No. 4 on October 6, 2017 in the amount of
$20,800.00.
WHEREAS, further amendment is necessary to reimburse the Architect for professional services for the
project as part of the design services agreement; and
NOW THEREFORE, City and Architect, acting herein by and through their duly authorized
representatives, enter into the following agreement which amends the Contract:

Article 1.1, SECTION I — SERVICES OF THE ENGINEER, the following paragraph is added to read as
follows:
°Provide Construction Documents and Construction Administration Services as required for the
facility. The scope of services is described in the Elements of Architecture, Inc. letter dated May
16, 2018, Subject: Proposal for A/E Services — Tactical Center — KXAS Additional Services —
Additional Construction Administration Phase Services — Additional Services 03 - REVISED,
attached hereto as Attachment"A" and made a part of this Agreement". This Amendment No. 5
increases The Contract by$72,000.00 including up to$0.00 in reimbursable expenses.
2.
Paragraph 1 of SECTION V-COMPENSATION TO ARCHITECT, is amended as follows:
"The total compensation for all of the assignments to be performed by Architect as described in
CHARACTER AND EXTENT of ARCHTECT'S SERVICES hereof shall be a fee not to exceed of
$295,062.00.'

effect.

Tactical Center- KXAS Additional Services—Additional Construction Administration Phase Services—
Additional Services 03 REVISED
Dear Ronnie,
Per our discussions, we are providing this proposal to you for consideration of services for the CA phase for the
referenced project based on exceeding the Additional Services 02 through April 2018 as well as our anticipated
services through to the end of August 2018 as the new completion schedule for the contractor. Add Services
02 was based on completion date of November 2017. This proposal,Add Services 03, reflects actual hours thru
April 2018 plus our estimate based on the precedent of the project through construction completion. The
proposed fees are proposed as follows:
Architectural Services thru April 2018
(based on actual hours spent over contracted amount) $ 36,000.00
Architectural Services May 1 thru August 2018(estimated) $ 20,500.00
Civil Engineering $ 6,000.00
MEP Engineering $ 3,500.00
Structural Engineering $ 6,000.00
Total Labor Fee S 72,000.00
The tasks included within our estimated fee are as follows:
OAC meetings
Shop drawings still outstanding
RFIs
Review of Pay Applications
Punchilsts for remaining phases and site
TAS Walk through with RAS
Project Close-out
If this proposal meets with your approval,we would anticipate execution of an amendment to our existing city
