Addendum No_ 11 CONTRACT NO.
Statement of Work
This Addendum No. 11 - Statement of Work (SOW) is made as of this ] day of
August 2018, under the terms and conditions established in the MASTER
PROFESSIONAL SERVICES AGREEMENT between the City of Fort Worth ("City") and
Mackenzie Eason & Associates, LLC ("Consuitani°), the same being Fort Worth City
Secretary Contract (CSC) No. 48121 (the "Master Agreement" or "MSA"), as renewed
by CSC No. 48121-R1.
This Statement of Work is made for the following purposes, consistent with the services
defined in the Master Agreement: recruitment and hiring of a new Regional-Mobility
Coordinator for the City of Fort Worih. In particular, Consultant shall perform...,h.ose

payment of Fees and Reimbursable Expenses, including tl,e structure of paLment.
invoicing therefor, shall be in accordance with the Master Professional Service
Agreement. The total Fee to beap id to Consultant for the Regional Mobility Coordinator

Section E. Other Provisions:
PROHIBITION ON CONTRACTING WITH COMPANIES THAT BOYCOTT ISRAEL
Consultant acknowledges that in accordance with Chapter 2270 of the Texas
Government Code, the City is prohibited from entering into a contract with a company
for goods or services unless the contract contains a written verification from the
company that it: (1) does not boycott Israel; and (2) will not boycott Israel during the
term of the contract. The terms "boycott Israel" and "ccmp.sny" shall have the meanings
ascribed to those terms in Section 808.001 of the Texas Government Code. By
signing this SOW, Consultant certifies that Consultant's signature provides
written verification to the City that Consultant: (1) does :oot boycott Israel; and (2.)
will not boycott Israel during the term et the SOS! or the (VISA.
Section F. Counterparts; Electronically Transmitted Signature:
This SOW may be executed in one or more counterparts, each of which when so
executed and delivered shall be considered an original, but such counterparts shall
together constitute one and the same instrument and agreement. Any signature
delivered by a party by facsimile or other electronic transmission (including email,
transmission of a portable document file (pdf) or similar image) shall be deemed to be
an original signature hereto.
