FTggyry PROFESSIONAL SERVICES AGREEMENT
This FIRST RENEWAL AND AMENDMENT OF FORT WORTH CITY
SECRETARY CONTRACT NO. 50002 is made and entered into by and between the City of
Fort Worth, a home-rule municipal corporation of the State of Texas ("City"), acting by and
through Valerie Washington, its duly authorized Assistant City Manager, and PetVet Relief
Services, PLLC ("Contractor"), a Texas professional limited liability company, each individually
referred to as a"party" and collectively referred to as the "parties."
WHEREAS, City and Contractor are currently parties to a Professional Services
Agreement for veterinary services, the same being Fort Worth City Secretary Contract
No. 50002 which was executed on November 27, 2017 with an effective date of October 24,
2017 (the"Agreement"); and
WHEREAS, the Agreement provides for an initial one-year term with options for up to
four additional one-year terms; and
WHEREAS, the parties now wish to renew the Agreement under the same terms and
conditions for a first renewal term, commencing October 24, 2018; and
WHEREAS, the parties also wish to amend the original Agreement to provide additional
services pursuant to the Agreement.
NOW, THEREFORE, the City of Fort Worth and Contractor do mutually covenant and
agree as follows:
1.
Pursuant to the Agreement, City and Contractor hereby agree to renew and extend Fort
Worth City Secretary Contract 50002 for a first Renewal Term of one (1) year, beginning on
October 24, 2018, and ending October 23, 2019.

Exhibit "A" Scope of Services. The services included in the Agreement as Exhibit "A"
shall be amended to allow for a third trimester pregnancy fee as follows:

good surgery candidates at all times. No in heat fees will be assessed._I' 'rd trimester

Exhibit "B" Payment Schedule. The payment schedule included in the Agreement as
Exhibit "B" shall be amended to allow for a third trimester pregnancy fee as follows:

4.
All terms and provisions of City Secretary Contract 50002 shall remain in full force and
effect throughout the first Renewal Term unless specifically amended herein.
