To Whom It May Concern:
This document serves to memorialize the agreement between City of Fort Worth-Police Department("Subscriber")and West
Publishing Corporation("West")regarding the modification to the Special Offer Amendment to Subscriber Agreement for
CLEAR Services("Amendment")currently in effect between Subscriber and West. Capitalized terms used,but not defined
herein,will have the meanings given to them in the CLEAR Subscriber Agreement and/or the Amendment.
Effective August 1,2018,this Amendment is under DIR code DIR-LGL-CALIR-02.
The Term of the Amendment will be extended through July 31,2021.
The Period(s)and the Monthly Guarantee(s)will be:
• The Period 5(August 1,2018 through July 31,2019)Monthly Guarantee will be$4,932.
• The Period 6(August 1,2019 through July 31,2020)Monthly Guarantee will be$5,178.
• The Period 7(August 1,2020 through July 31,202 1)Monthly Guarantee will be$5,437.
You may opt-out of Period 6 or Period 7 by providing us with at least 60 days prior written notice of your intent to terminate the
agreement effective upon the completion of the then-current period.
Except as expressly provided herein,all other terms and conditions of the Amendment will remain unchanged. Please have this
document executed by an authorized representative of Subscriber and returned to West,on or before July 27,2018 at 5:00 p.m.
