Inc },�c�� NGINE R) made and entered into City Secretary Contract No. 43893, �-, (the 'CONTRACT) which was authorized by the City Council by M&C C-25936 on the 23rd day of October, 2012 in the amount of $563, 730 . 92; and Amendment No. 1 which was authorized by the City Council by M&C C-26486 on the 7th day of October, 2013 ; Amendment No. 2 which was administratively authorized by the City on the 8th day of June, 2015; and Amendment No. 3 which was authorized by the City Council by M&C C-27775 on the 21st day of June, 2016; and Amendment No. 4 which was administratively authorized by the City on the 6th day of February, 2017; and WHEREAS, the CONTRACT involves engineering services for the following project: Chapel Creek Bridge over Interstate Highway 30 WHEREAS, it has become necessary to execute Amendment No. 5 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1. Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated April 11, 2018, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $45, 905 . 50. 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $967, 522.42 . 3 . The following is a brief description and cost for the additional services to be provided on this project: I. Project Management: Teague, Nall and Perkins will be required to perform additional project management services to coordinate with utility owners, City of Fort Worth and TxDOT. TNP will. 1. Status Reports: The Engineer shall provide the State with a status report for all utility adjustments on a monthly basis. The State will provide the status report format to the Engineer. 2. Review Payment Request: The Engineer shall review all payment requests for conformance with the utility estimate and verify the work has been performed. Fee: $5,000.00 II. Traffic Control Plan Revisions at Sonic Driveway Teague, Nall and Perkins will revise traffic staging to provide a temporary driveway at the Sonic entrance along Chapel Creek Blvd. 1. Meet with business representatives and City and coordinate with TxDOT 2. Revise TCP plans. Construction Phase Services ENGINEER will support the construction phase of the project as follows. 1. The ENGINEER shall visit the project site at requested intervals as construction proceeds to observe and report on progress. 2. The ENGINEER shall review shop drawings, samples and other submittals submitted by the contractor for general conformance with the design concepts and general compliance with the requirements of the contract for construction. Such review shall not relieve the Contractor from its responsibility for performance in accordance with the contract for construction, nor is such review a guarantee that the work covered by the shop drawings, samples and submittals is free of errors, inconsistencies or omissions. The ENGINEER shall log and track all shop drawings, samples and other submittals. 3. As requested by the CITY, the ENGINEER shall provide necessary interpretations and clarifications of contract documents and review change orders. ASSUMPTIONS 8 submittal reviews are assumed. • 8 RFI's are assumed. • 5 Change Orders are assumed. Fee: $29,820.00 IV. Utility Coordination (Lump Sum) 1. Utility Conflict Review. As requested by the CITY, the ENGINEER will review utility conflicts determined in the field and coordinated utility adjustments or design changes to resolve the conflict. 2. Design Revisions. The ENGINEER will coordinate with the utility company to accurately locate the potential conflict and develop revised design plans to be executed through a Change-Order to resolve utility conflicts.
