WHEREAS, the City of Fort Worth (CITY) and EJES, Inc. ,
(ENGINEER) made and entered into City Secretary Contract No. 43806,
(the CONTRACT) which was authorized by M&C C-25848 on the 18th day
of September, 2012 in the amount of $193,110. 00; and
WHEREAS, the CONTRACT was subsequently revised by Amendment
No. 1 in the amount of $119,387 .00 which was authorized on July
22, 2014 (M&C-C-26877) , Amendment No. 2 in the amount of $48,942 .00
which was administratively authorized on October 9, 2015 and
Amendment No. 3 in the amount of $57,248 . 00 which was authorized
on August 29, 2017 (M&C C-28354) ; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
Sanitary Sewer Rehabilitation Contract 91; and
WHEREAS, it has become necessary to execute Amendment No. 4
to the CONTRACT to include an increased scope of work and revised
fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and
through their duly authorized representatives, enter into the
following agreement, which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated August 23, 2018 ,
a copy of which is attached hereto and incorporated herein. The
cost to City for the additional design services to be performed by
Engineer totals $53,328 . 00.
2 .
Article II of the CONTRACT is amended to provide for an
increase in the fee to be paid to Engineer for all work and services
performed under the Contract, as amended, so that the total fee
paid by the City for all work and services shall be an amount of
$472, 015 . 00 .
All other provisions of the Contract, which are not expressly

amended herein, shall remain in full force and effect.

Additional Design and construction Services
Dear Mr. Chaubey:
Per your request we are pleased to submit this Amendment No. 4 for additional design services required to
complete the above referenced project. The requested modifications will be outside of our scope of services
as set forth in City Secretary Contract No. 43806, therefore this Amendment No. 4 is being developed to
further identify these services and summarize the associated costs.
The additional services required to prepare final construction documents and construction services will
include the following:
• Project Coordination for Sanitary Sewer improvements CPN-01957, Westcliff Drainage improvements CPN-
01669 and Asphalt Rehabilitation CPN-0157
• Construction Services
• Additional Prints of construction plans and specifications
We are requesting your assistance in the execution of this Amendment No. 4 in order for us to expeditiously
perform these services.
The attached EXHIBIT I, outlines the additional tasks, personnel and man-hours we anticipate will be
required to make the requested revisions. The total fee for these additional services will be an amount not
to exceed $53,328.00.

The total contract amount to date, including amendments, is as follows:
Original Contract Amount = $193,110.00
Amendment No. 1 = $119,387.00
Amendment No. 2 = $ 48.942.00

New Contract Amount = $472,015..00
The City MWBE goal for this contract is 22%. The total amount paid to MIWBE (Arredondo, Zepeda, &
Brunz, LLC) on total contract amount of$472,015.00 is $119,762.00, which is 25.37%; therefore
meeting the City MWBE goal of 22%.
