COUNTY OF TARRANT S
WHEREAS, the City of Fort Worth (City) and Stream Water Group
Inc. , (Engineer) made and entered into City Secretary Contract No.
33661, (the Contract) which was authorized by the City Council by
MSC C-21502 on the 13th day of June 2006,and
WHEREAS, the Contract involves engineering services for the
following project:
Water and Sanitary Sewer Replacement Contract 2005, WSM-A.
WHEREAS, it has become necessary to execute Amendment No. 1
to the said Contract to include an increased scope of work and
revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and
through their duly authorized representatives, enter into the
following agreement which amends the Contract:
1.
Article I of the Contract is amended to include the
additional engineering services specified in a proposal letter
dated December 21 20101 a copy of which is attached hereto and
incorporated herein. The cost to City for the additional design
services to be performed by Engineer total $4,395.00.

Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work
and services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $154,941.00.

All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Dear Mr.Chaubey:
The following is a summary of additional work requested for Amendment No. 1 to the above referenced project,per
recent design meetings with City of Fort Worth officials.
Amendment No. 1:
Redesign 791 linear feet of 12" Water line at Beaumont Street(2 Plan and Profile Sheets)due to a conflict with the
proposed 7X5'Storm Drain system along the existing waterline by another consultant.
Update the Sewer Plan and Profile sheet to reflect the redesign of waterline.
Update three(3)supporting sheets(Cover Sheets, General Notes, Location Map)and
Addition of Standard Details(3 sheets)
Amendment no. I (See attached Exhibit"]" _ $4,395.00
The current status of the contract between Stream Water Group,Inc and City of Fort Worth for the Water and Sanitary
Sewer Replacement Contract 2005,WSM-A is as follows:
Original Contract Amount= $ 150,546.00
Amendment No. 1 Amount= $ 4,395.00
Total Contract Amount including Amendment No. 1= $154,941.00
Please process Amendment No. 1,to the Water and Sanitary Sewer Replacement Contract 2005,WSM-A in the
amount of$4,395.00.
