HomeMy WebLinkAboutContract 33801 t AMENDMENT NO. 1 STATE OF TEXAS $ CITY SECRETARY CONTRACT NO. �1 COUNTY OF TARRANT $ WHEREAS, the City of Fort Worth (CITY) and MAXIMUS, Inc. , (CONSULTANT) made and entered into City Secretary Contract No. 32246, (the AGREEMENT) ; and WHEREAS, the Agreement involves professional services for the following project : Cost analysis of infrastructure related plan review, inspection and CFA activities WHEREAS, it has become necessary to execute Amendment No. 1 to said Agreement to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Consultant, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the Agreement: 1. Section 1 of the Agreement is amended to include the additional consulting services specified in a proposal letter dated December 16, 2005, a copy of which is attached hereto and incorporated herein. The cost to City for the additional services to be performed by Consultant total $3, 260 . 00 . 2 . Section 2 of the Agreement is amended to provide for an increase in the maximum fee to be paid to the Consultant for all work and services performed under the Agreement, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $19, 930 . 00 . c T.0 �JRIGINAL 3 . CONSULT By: APPROVED TO FORM AND LEGALITY: Amy Ram Assistant ity Attorney Page 2 or 2 3 :e December 16, 2005 Christa Sharpe Assistant Director Transportation and Public Works City of Fort Worth, Texas 1000 Throckmorton Fort Worth, Texas 76102 Dear Ms. Sharpe: As discussed in the joint departmental meeting on October 14, 2005, the City asked MAXIMUS to perform several additional tasks related to the Civil Plan Review User Fee Study. These tasks were not included in my proposal letter dated July 7, 2005. In response to my request for additional compensation, you asked me to submit a request for a change order. Since that meeting, both City staff and MAXIMUS consultants have spent considerable time adjusting expenditure information, time estimates, and reviewing options and discussing fee structure alternatives. Since we are now close to completing all the tasks, I prepared a single change order request that will cover all tasks to be completed by December 31, 2005. Detailed below are those additional tasks MAXIMUS completed after presentation of the DRAFT Civil Plan Review User Fee Study on October 14, 2005. Some of these tasks are included in the original scope of services and will be billed under the original contract scope as Task 7 (Presentation of Results to City Officials/Developers/Other Interested Parties). The City has already paid MAXIMUS for Tasks 1 through 6 as defined in my original proposal letter. Tasks included in original scope of services (will be billed as Task Revise Water Department data to include additional time for various staff members on fee related activities defined in the draft report and incorporate new salary data for affected positions. (2 hours) Prepare PowerPoint and make presentation of results to City officials (department heads) — (8 hours) Revise TPW models to include new position added after completion of draft report. (0.25 hour) Revise PowerPoint presentation (FINAL REPORT) to include adjustments requested by the department heads in the meeting on December 9, 2005 (estimated 6 hours). Christa Sharpe December 16, 2005 Page 2 Additional Charges (requested as a change order to the original contract) Revise Costing Data in DOE, TPW, Water, and Sewer to include separate fee calculations for four categories of inspection charges: Water, Sewer, Streets, Storm Drains. (4 hours) Review and analyze new databases (2 databases provided) for DOE charges related to plan reviews, inspections, and materials testing; prepare summary analysis (graphs) for . further discussion of plan review fee structure development. (8 hours) Develop additional cost models for Civil Plan Review to include "Processing Fees" for TPW, DOE, Water, and Sewer. (4 hours) Update salary information and DOE multiplier based on projected 2005-06 salary and benefit levels for all cost models (20 separate cost models). (4 hours).
