HomeMy WebLinkAboutContract 34092-A4 (2)STATE OF TEXAS S COUNrY OF TARRANT S AMENDMENT NUN�ER 4 CITY SECRETARY CONTRACT NO . �'O"12� 1� WHEREAS, the City of Fort Worth (CITY) and Teague Nall and Perkins, Inc., (ENGINEER) made and entered into City Secretary Contract No. 34092, (the CONTRACT) which was authorized by the City Council by M&C C-21722 on the 26th day of September, 2006; and subsequently revised by Amendment Number 1, administratively authorized on the 22nd day of February, 2008; and subsequently revised by Amendment Number 3, administratively authorized on the 12th day of August 2010; and WHEREAS, the numbering of the amendments inadvertently omitted Amendment No. 2 and, therefore, no Amendment No. 2 exists; and WHEREAS, the Contract involves engineering services for the following project: South Pipeline Road at Post Oak Channel Bridge Replacement (Project No. 00258; DOE No. 5406); and WHEREAS, it has become necessary to execute this Amendment No. 4 to said Contract to include an increased scope of work and revised maximum fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in the proposal letter dated October 6, 2011, copies of which are attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $6,800.00. 2. Article II of the Contract is amended to the maximum fee to be paid to Engineer performed under the Contract, as amended, sc the City for all work and services sha: $118,571.00. Department of Transportation and Public Works City of Fort Worth 1000 Throckmorton Street Fort Worth, Texas 76102-6311 RE: SOUTH PIPELINE ROAD AT POST OAK CHANNEL BRIDGE REPLACEMENT CITY PROJECT NO. 00258 TNP No. FTW 06290 Dear Mr. Weiss: Teague Nall and Perkins, Inc. is submitting this fee Amendment to the above referenced contract based on additional effort required by the City but not included in the original scope of worl� The following is a brief description and cost for the additional services provided on this project: Maintenance of Buzzsaw files during AdvertisemenbBidding Period This work involves uploading, maintenance and coordination of the files on Buzzsaw during the advertisement/bidding period. Any questions from Contractors during the bidding period must be logged and maintained in a document on Buzzsaw along with an accurate updated plan holders list. This work also involves assembling the bid proposal using the new CFW bid-tools program which is a specific spreadsheet workbook that allows Contractors to access the bid proposal and fill it out on the Buzzsaw website. ■ Maintenance of Buzzsaw Files ...........................................................$ 1,800 Addifional Services for Switching to New Specifications Previously, TNP had submitted contract document specifications (old format) with the final plan submittal. This work involves additional effort to prepare new contract documents that conform to the specifications recently adopted by the City of Fort Worth. Using the new specifications will require us to revise the proposal bid form to match the new bid item descriptions and specification numbers associated with each bid item.
