L5 (No M&C Required) COUNTY OF TARRANT S WHEREAS, the City of Fort Worth (City) and Lockwood, Andrews & Newnam, Inc. , (Engineer) made and entered into City Secretary Contract No. 28790, (the Contract) which was authorized by the City Council by M&C C-19585 on the 13th day of May, 2003 and subsequently revised by six previous amendments; and WHEREAS, the Contract involves engineering services for the following project: Sanitary Sewer Rehabilitation Contract LXVII (67) . WHEREAS, it has become necessary to execute Amendment No. 7 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated April 19, 2006, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $24,280.00. 2. Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $458,740.00. r � � '4T`JU°�W9 RIGlNIVIA 3. A LEO A DALY COMPANY April 19,2006 Mr. Liam Conlon City of Fort Worth Engineering Department—Engineering Services 900 Monroe Street, Suite 201 Fort Worth, Texas 76102 Re: Amendment No. 7 Survey and Engineering Design Services for Sewer Line Replacements Sanitary Sewer Rehabilitation Contract LXVII (67) DOE #4800 Water Project No. PW53-060530177970, and Sewer Project No. PS58-070580175790 Dear Mr. Conlon: Per your request, this Amendment No. 7 has been prepared for the performance of additional survey and engineering design services with respect to the above referenc d project. The work specifically relates to the development of additional construction documents for replacement of the existing L-3246 sewer line located in the Bailey Industrial Park, and specifically between Tillar and Cullen Streets, immediately east of Foch Street (see attached exhibit). The additional work will include meetings and negotiations with Mosites Rubber Company, who is the main property owner in the area, for the purpose of identifying alternative alignments and acceptable easement locations. This will be followed by the performance of topographic surveying of the identified future alignment locations. Construction plans will then be developed for the replacement of the existing L-3246 line with new sewer lines and services. The project intent will be to extend the new sewer lines and services to the existing L-3245 sewer line located in Tillar Street, to the south. Design improvements are to be incorporated into the Part 3 plans for the Contract 67 project that have been previously prepared to a pre-final level of completion. In addition to the above listed work, LAN will break out the replacement design for the existing M-132 sewer main in White Settlement Road, to be performed as an emergency service. Recent television inspection has revealed that this segment of 24" sewer main is severely deteriorated and in need of immediate replacement. The plan sheets for this area will be revised accordingly so that the work can be performed independently of the other segments. A corresponding traffic control plan for this segment will also be developed. LAN will coordinate with the City as necessary to transfer the applicable plan sheets to this other project. The remainder of the Part 3 plans will be modified to coordinate the separation from the adjacent project. Based upon the work to be performed as described above, a lump sum fee addition of$24,280 is hereby requested. A summary of the contract values affected by the amendments including this Amendment No. 7 as proposed herein follows at the bottom of this letter. Thank you for your consideration.
