AMENDM�NT TO CITY SECRETARY
CONTRACT NO. 33725
WHEREAS, on June 1, 2006 the City of Fort Wo�th ("City") and SafeHaven of Tarrant
Cotuity. ("Coiitr�ctor") n�de and entered into City Sec�etaiy Contract �to. 33725
(the "contract");
WHEREAS, the contract provided funding to Contxactor pursuant to a grant received by City
fro»� the Uiuted States De�aitinent of Housing a�id U�ban Develo�i��ent ("I�UD") under Subtitle
B of Title IV of the McKinney-Venta Hameless Assistance Act, as amended, (42 USC 11371 et
seq.) in connection with the Emergency Shelter Grants ('BSG") program, Pi•agram No. S-O6-
MC-48-0006, for Year XXXI;
WHEREAS, one of the national objectives of the ESG program is ta benefit low and
moderate incon�e citizens (the "National Objective"};
WHEREAS, tl�e �orit�act ex�i�°es by its ter�ns on May 31, 2007;
WHEREAS, it the mutual desire af �ity and Contractor to amend City Secretary Contract
No. 33725
NOW, THEREFQRE, City, whose address is 1000 Throckmorton St., Fort Wo��th TX
76102, acting by and through Dale A. Fisseler, its duly authorized Assistant City Manager, and
SafeHaven af Tarrant County, whose address is 6818 Manhattan Drive, Suite 105, Fort Worth,
TX 76120 ("Contractor"}, acti�g by and througli Ms. Mary Lee Hafley, its duly autharized
Piesic�et�t/CEO, do hereby agree as follows:

4. Reporting Procedures tlus sectiou is auiended to be and read as follows by add'uig new
subsection f.
f. Contractor shall participate in a local Cantinuum af Care's Homeless I�Nlanagement Infarmation
Systern ("HMIS") to ensure that the Pra�i�a�r► meets tHe Federal gaal tl�at localities collect an
array of unduplicated data on th� number and characteristics of homeless persons. Continuums of
Care are local bodies that plan for and coordinate hotneless sei-vices. �ND's standards for
p�rticipation in a local HMIS are set forth in the Federal Register, July 30, 2004. The local
Contiriuum of Ca.��e is Fart Wai�tl�/Arlington/T��°�t Cour�ty CaC (TX-601}.
