WIIEREAS, on June 1, 2006 the City of Fort Worth ("City") and Mental Health Mental
Retardati�n of Tazy�a.i�t Cc�ui�ty. ("Contractor") tnade ai�d �ntered 'nito City Secreta.ty Contract
No. 3391 S
(the "contract");
WHEREAS, the contract provided funding to Contractor pursuant to a grant received by City
fiom the Uiuted States Depa.i�tme�it df Housing ��d Uiban Developm�nt ("HUI�") under Subtitle
B of Title IV af the McKinney-Vento Homeless Assistance Act, as amended, (42 USC 11371 et
seq,) in connection with the Emergency Sk�elter Grants ("ESG") program, Program No. 5-06-
MC-48-0006, for Year XXXT;
WHEREAS, one of the national objectives of the ESG program is to benefit ldw and
moderate income citizens (the "National Objective");
W�I�It�AS, the Conti•act exp�res by its terms on May 31, 2007;
WIIEREAS, it the mutual desire of City and Contractor to amend City Secretary Contract
No. 33915
NOW, THEREFORE, City, whose address is 1000 Thrackmorton St., Fort Woi�th TX
76142, acting by and tluough Dale A. Fissele�•, its duly authorized Assistai-�t City Manager, and
Mental Health Mental Retardation of Tarrant Couniy, whose address is 3840 Hulen Towers
North, Fort Worth, TX 76107("Contr�ctar"}, actuyg by and through Nfr. Jui� McDegli�ott, its duly
authorized Executive Di�ector, dv here'dy agree as follows:

£ Contractor shall participate in a local Continuum of Gare's Homeless Management Information
System ("HMIS") to ensure that the Program n��ets the Federal gc�a1 that localities collect an
array of undupl�cated data on the number and characteristics of hoineless persons. Continuums of
C��e are loc$1 Uodies tliat plan for �d �o��du�ate h�n�eless services. �IUD's standasds for
participation in a local HMIS are set forth in the Federal Register, July 30, 2004. The local
Continuum of Care is Fai�t Worth/Arlington/Tarz•ant County CoC (TX-601).
